    On Site Laser Safety Check List

    Class IV Check List: Start-up, Procedures:


    * 1. Check / inspect - projector mechanical aperture shutters are in place * 2. Check / inspect - that projector & laser safety labels are visible and in place * 3. Make sure / inspect - the staging area is clear of personnel. * 4. Assure / inspect - that beam blocks and shutter are in the OFF position * 4. Turn on - Laser power breaker, key switch or Module. * 5. Test beam - is terminated in projector (Beam Block Test) * 6. Recheck / inspect - there are no personnel in staging area * 7. Send out aiming beam at lowest power with laser controller (AT ALL TIMES) * 8. Verify that all beams are terminated at assigned targets * 9. Verify there are no aerial beams (Exception NTAM LOD Government Approval) * 10. Check / inspect for back reflections (Beam Splatter) * 11. Make Sure / inspect - All remote targets are properly terminated & secure
