Amending Agreement

THIS AGREEMENT dated as of Effective Date of Amendment (ie. July 1, 2000) between
Name of Party 1 as in Original Agreement (the “1st Party”) and Name of Party 2 as in
Original Agreement (the “2nd Party”).

WHEREAS by a Describe Agreement being Amended (ie. Consulting Agreement) dated Date of
Agreement which is being Amended (the “Original Agreement”) the 1st Party and the 2nd Party
entered into an agreement relating to Brief Description of Nature of Original Agreement (ie. the
Consulting Services to be provided by the 1st Party to the 2nd Party);

AND WHEREAS the 1st Party and the 2nd Party wish to amend the Original Agreement in
accordance with the terms and conditions hereof;

NOW THEREFORE in consideration of the premises and for other good and valuable
consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree as
follows:

1.The parties agree that the recitals above are true and correct in all material respects.

2.The parties agree that the Original Agreement is hereby modified and amended as follows.

List all the amendments to the Original Agreement in Detail

3.The parties hereby confirm that all other terms and conditions of the Original Agreement are in
full force and effect, unamended except as expressly provided in this Agreement.

4.This Agreement may be executed in one or more counterparts, each of which when so executed
shall be deemed to be an original and such counterparts together shall constitute one and the same
instrument.

5.The parties acknowledge that this Agreement may be negotiated and transmitted between the
parties by means of a facsimile machine and that the terms and conditions agreed to are binding
upon the parties. Upon the Agreement being accepted, copies of the facsimile will be validated by
both parties forthwith.
