GENOGRAM NARRATIVE

On the maternal side of patient, his grandmother Merlita who is 45 years old, is still alive and is known to have a heart problem. The older brother of Merlita, named Lito died of Bronchopneumonia at 56 years old. On the other hand, patients grandfather Pepe who is a soldier, died at the age of 65 in an encounter in Maguindanao. No siblings can be recalled by the mother of the patient. Merlita and Pepe had only one child and her name is Jocelyn, the patients mother. Jocelyn recalled being hospitalized due to a heart problem, but cannot specifically name her condition. She had a history of two abortions. The first one is already at its 5th month and was aborted when she fell of the swing twice. The second one happened on its 3rd month and the mother had a superstitious belief that it was because she failed to eat the chicken of their neighbor. On the paternal side of patient, his grandmother Delia, 58 years old, is the eldest among 4 siblings. She is currently experiencing osteoarthritis and doesnt have any other known disease condition. The 2nd born in the family is Romy, 55 years old followed by Jose, 37 years old and the youngest, Baby who is 25 years old, all of them have no known disease condition. On the other hand, patients grandfather, named Fortunato died at the age of 44 years old on an on-going surgery due to ruptured appendicitis. His brothers, Danilo 40 years old and Pablo 36 years old both have a history of pneumonia. Delia and Fortunato was blessed with 5 children. The eldest is the patients father, named Retchie, Sr. who is 32 years old and had a history of hospitalization due to bronchopneumonia. Edmond, the 3rd child of the family had a history of recurrent convulsion during his childhood years and is often brought to the hospital for treatment. The other siblings named Carolina, 29 years old, Jessa who is 19 years old and Jedelyn, 17 years old, have no known disease condition. Jocelyn and Retchie, Sr., the patients parents have 4 children. The patients eldest brother, named Richard who is now 12 years old was born prematurely and was also hospitalized due to cystosomiasis. Rutchielyn, the patients older sister who is 12 years old, is asthmatic and had previous hospitalizations due to urinary tract infection (UTI). The 3rd child of the family is our patient, Retchie, Jr, is also known to be asthmatic, had a history of UTI and had recently undergone ORIF due to fractured bone which he got from an accident. His younger brother, Ricky is 4 years old and was also hospitalized due to UTI.
