IEP Activity Description Complete Parts 1 and 2 and submit both forms on the due date.

Part 1: Identify and Summarize Components of an IEP. Utilize the IEP provided with this assignment. Review the document and record the answers to the following: 10 Points 1. 5. Briefly describe the childs Present Levels of Academic Achievement and Functional Performance. Include what the child can and cannot do. On his present level of academic achievement: Cosmo can hold a book on his own and likes to have books read to him. He can identify objects but is having difficulty with consistently identifying colors verbally. Cosmo can hold a pencil according to his age. He can make vertical and horizontal lines. He is working towards cutting across a piece of paper. Cosmo is okay in the area of pre writing. He is consistent when counting to 10. He is working on sorting objects by color or shape. He has not been in a formal classroom setting yet. On his functional performance: Cosmos gross motor skills are within the age-appropriate limits. He can snip with scissors and hold a crayon with a digital grasp. He can jump, run, hop, and walk up and down stairs. Cosmo likes to help and please others. He is impulsive and/or aggressive at times but can be redirected. He cannot sit still for long period but is working on it. Cosmo can put and take off his own clothes by himself. As long as the clothing opens from the front and has elastic waistbands. Cosmo is able to use an open cup with some spills, but he uses a sippy cup. Cosmo is able to use the spoon and fork with minimum spills.

6. List at least 2 goals written for this child. 1. Cosmo will sort objects by either color or shape using no more than 4 various colors or shapes. He will master this goal when he can do this task independently with 90% accuracy 3 out of 4 opportunities per week over nine consecutive weeks. If during the IEP year the student turns 16, and he is not present the service coordinator must review the IEP with the student and obtain his signature and date of the review.

Part 2: Planning Intervention based on Goals and Objectives. Use the Intervention Plan form and submit it along with the above Part 1 on the due date. 10 points Directions 1. Choose 2 goals targeted in the IEP and record them in their entirety on the Intervention Plan form. You may use the two you identified in Part 1. 2. Develop and record specific activities that will provide the child the opportunity to practice these skills. 3. Record any specific materials you would need or any changes to the environment that may be needed. This could include special seating, allowing more time for the child to complete the activity, assistive technology or communication devices (pictures, switches, voice output tools.) 4. Record how you will monitor and assess progress. How will you know when the child has mastered this skill? How will you record the data? Checklists and photos can be used, however, there are many ways to track progress. Be creative! Remember to utilize your text as well as supplemental materials provided on Blackboard.

Monitoring Progress (how will you document the childs progress?) I will make anecdotal notations and use tally sheets work sampling. I will take photos while he is working on these activities.

Cosmo will sort objects by either color or shape using no more than 4 various colors or shapes. He will master this goal when he can do this task independently with 90% accuracy 3 out of 4 opportunities per week over nine consecutive weeks. Cosmo will independently sort objects by color or shape. Tally sheets work sampling which could include a photo anecdotal notation.

Hands-on sorting objects by color. Cosmo will put the objects in each basket according to color. Cosmo will do this activity for about 20 minutes 4 times a week. Basket comes with different color and chipboard socks. Socks have a variety of colors and patterns.

Sorting objects by shape. Bright buttons with different geometric shapes. He will sort the different shapes unto a colored mat. Cosmo will do this activity for 20 minutes 4 times a week.

Cosmo will count fun, small objects from 1 to 8. He will do this for 20 minutes 4 times a week.

I will make anecdotal notations and use tally sheets work sampling. I will take photos while he is working on these activities. Will have moms report on how much progress he made at home.

The routine will change a little because he will be listening to a couple of counting songs during activity time, instead of music time. A bag of math counters will be sent home also in case mom wants to work with him at home.

When we go to circle time I assign a number on the carpet for each student. I will be assigning Cosmo a number from 1-8 for more practice I will make a notation of that each time.
