Client Identification For the second semester CHN exposure of SY 2012-2013, the level IV nursing students utilized the

Intensive Nursing Practice (INP) for its focus. By this period, student-nurses are already equipped with basic knowledge regarding Community Health Nursing most specifically the integration of the nursing process to the care of community, family, and individual. This time, each group per Clinical Instructor was assigned to a permanent Barangay for the whole semester from which they are tasked to create a community comprising their chosen clients. For our group under the supervision of Mrs. Fritzie Camacho, RN, MAN, we were assigned in Pob. 3, Tagbilaran City. Since we are the first group to be exposed in the area, the task of identifying prospective clients felt on us. On our first day of exposure on November 13, 2012 after we had our courtesy call to the barangay officials, we had identified our clients. The selection of clients was based on the list provided by the brgy. health workers taking into consideration their health status and location. Out of the 20 prospects, the final list was trimmed down to 15 clients together with their family. Most of our clients are from Hontanosas & Matig-a street respectively. After informing them of our purpose and when rapport was achieved by both parties; health assessment was done utilizing the different tools provided. Pertinent data were then recorded in our individual CO notebook and proper endorsement was made for the next student nurse assigned.
