AFFIDAVIT We, Katrina Salcedo, Princess Quennie Aban, and Justine Kenneth Mapalo 4th year Highschool students

of Northwestern University, after having sworn in accordance with law, do hereby depose and state THAT: 1. Sometime on August 20, we have given an aggregate amount of money amounting to four thousand nine hundred forty pesos (P2000, P1840 and P 1100, respectively) and our respective assessment forms to our adviser Mrs. Leilanie Guieb for the purpose of paying our first grading tuition fee at the Finance Office because she expressly said to us that if we are not able to pay yet our tuition fee she can do the favour for us; After she received the said money and forms, she stapled it and kept it in her drawer; We have taken the test confidently and assuming that we are already cleared and have paid so that our permits are in the hands of our adviser and at the same time our proctor during the first grading exams. Sometime in October 15, we went to the Finance Office for us to pay our 3 rd grading tuition fee and that we were informed by the Finance Staff that we still have not paid for the first grading period; Upon learning, we went to find our adviser and saw her at the Student Center where a program was being conducted. We attempted to ask for our receipt but she seems not to see us and just ignored us. The following day, Katrina Salcedo texted her asking for our receipt however, she replied that she is absent that day; Consequently, Ms. Salcedo reported the matter to the Vice President for Finance.
