AMENDMENT OF PROMISSORY NOTE

This AMENDMENT made and executed in the Municipality of Carmen, Surigao del Sur, on this _____ day of ________________ 2007 by _________________________ and _________________________, both of legal age, Filipino citizen, with residence and postal address at ____________________________, Surigao del Sur, hereinafter referred to as BORROWER, in favor of the RURAL BANK OF LANUZA (Surigao del Sur), INC ., a banking corporation organized and existing under Republic Act No. 720, as amended, with main office at National Highway Pob. Carmen, Surigao del Sur, Philippines, hereinafter referred to as CREDITOR in the PROMISSORY NOTE establishing the indebtedness of the above mentioned borrowers and particularly covered by LOAN ACCOUNT NO. _______ in the amount of P __________ shall be and is hereby amended as follows: 1.
