Sub: Conversion of Canadian CPL into Indian CPL, with issuance of Single
engine endorsement with single engine IR rating and Multi Engine endorsement
with Multi Engine IR rating & FRTOL in respect of the undersigned.

Please find the following documents enclosed for the issue of Indian CPL (Single-
engine on C-172 and Multi-engine on PA-34-200),SEIR,MEIR & FRTOL in respect
of the undersigned.

1. Prescribed application format for issue of CPL and IR duly filled.

2. Bankers Cheque No.’6545768’ dated 11/ 05 / 09 for Rs.25,000/-, issued by
THE FEDERAL BANK LTD Chennai(any nationalized bank)
3. Philippines CPL,IR, Class I and Attested Pilot log book.
4. Class 10th Mark List & verification in original and copy.
5. Class 12th Mark List, & verification in original and copy.
6. Passport copy
7. License Verification letter from Civil Aviation Authority of the singapore, in
original & Copy.
8. Indian COP-RTR No. RTR (P) 2312/09-COP valid till 31-02-2010, in original
and copy.
9. Original and Copy of Indian Class I Medical certificate CA 35
10. Original and Copy of Indian Class I Medical Assessment.
11. DGCA Pilot License examination result cards (Air Reg+Composite), in
original and copy.
12. CA 39 form for five years in original & copy.(Note: it should be separated in
months, please click the links and save the form from victortango.in. The form
is different from the CA 39 form issued by dgca, but only victortango forms
are accepted, oor else will be rejected because of not listing hrs flown hours
monthwise . This is required for both 5yrs and 6months)
13. CA 39 form for preceding six months in original & copy.
14. Certificate of attendance for Commercial Pilot Ground Course in original &
Copy.
15. Certificate of attendance for Equipment Qualification Course for PA34-200 in
original & Copy.
16. Certificate of attendance for Instrument Ground Course in original & Copy.
17. General Flying test by day on single engine C-172, within 6 month original &
Copy.
18. General Flying test by night on single engine C-172, within 6 months original
& Copy.
19. IR Test report with two approaches on single engine C-172
20. General Flying test by day on PA34-200, within 6 months in original & Copy.
21. General Flying test by night on PA34-200 within 6 months in original & Copy
22. General Flying test by night on multi engine PA-34 200, within 6 months
original & Copy
23. IR Test report with two approaches on multi engine PA-34 200
24. Flight Check Test report for Issue of Rating on PA-34 200 & C-172.
25. Flight Check Test report for Issue of Instrument Rating on PA-34-200 &
C-172.
26. Airman Test Report from CAAP (Philippines) of theoretical exam conducted
for multiengine rating on PA34-200

27. Certificate of more than 15 hours flight as PIC within 6 month in original &

28. Certificate of more than 5 hours flight as PIC with more than 10 Take

29. Certificate of more than 5 hours of IR flight within six month in original &
copy.
30. Certificate of more than 50 hours X country flight as PIC in original & copy.
31. Certificate of more than 40 hours of IR time in original & copy
32. Certificate of more than 10 hrs of training in Multiengine aircraft in original &
copy
33. Certificate of Simulator Training & Simulator Type Certification in original &
copy
34. Certificate of zero accident/incident original & copy.
35. Certificate of 300 NM, 250 NM, 120 NM X-country day & night Flights
36. Certificate of 3 take-off/landings as PIC by day on C – 172 within 6 month
37. Certificate of 3 take-off/landings as PIC by night on C – 172 within 6 month
38. Certificate of 2 diff IR approaches on Single engine Instrument Check Ride.
39. Certificate of 3 take-off/landings as PIC by day on PA 34-200 within 6 month
40. Certificate of 3 take-off/landings as PIC by night on PA 34-200 within 6
month
41. Certificate of 2 diff IR approaches on Multi engine Instrument Check ride.
42. Certificate of Signal Reception Check Report for IR.
43. Total Flying hours Certificate
 44. Copy of Passport.
45. 6 copies of Photographs with name at the top.

Kindly issue the License and Ratings at the earliest and send original

Always remember to take to 2 copies of this Covering letter to dgca office, and don’t
stick the envelop which contains the documents, which you r going to submit, as dgca
will verify the documents at the receiving counter at the time of submission
One copy of covering letter should be kept along with the documents while the other
copy should be showed to the official; get it signed and acknowledged from him after
you submit the documents for cpl conversion. Please tie all the documents safely with
a thread.
