Teacher Work Sample 4 Assessment Plan

Unit Objectives:
Unit Objective 1
When given equations with multiples of 10 the students will be able to distinguish a pattern to simply multiplication with 90% accuracy. CC Math 5.NBT.2 Explain patterns in the number of zeros of the product when multiplying a number by powers of 10, and explain patterns in placement of the decimal point when a decimal is multiplied or divided by a power of 10. Use whole-number exponents to denote powers of 10.

Unit Objective 2
When given equations that include greater whole number multiplication problems that include multiplication properties the students will be able to generate solutions with 93% accuracy. CC Math 5.NBT.5 Multiply a whole number up to four digits by a one-digit whole number, multiply two two-digit numbers, using strategies based on place value and the properties of operations. Illustrate and explain the calculations by using equations, rectangular arrays, and/or area models.

Unit Objective 3
When given equations that include multiplication the students will be able to distinguish place value with 90% accuracy. CC Math 5.NBT.6 Find whole-number quotients of whole numbers with up to four-digit dividends and two-digit divisors, using strategies based on place value, the relationship between multiplication and division. Illustrate and explain the calculation by using equations, rectangular arrays, and/or area models.

Assessments:
Formative Assessments: Pre-Assessment During Assessment Post Assessment Summative Assessments: Chapter Test

Formative Assessments:
Formative assessments open doors for unit changes. Formative assessments showed whether students were ready to continue to new material. Formative assessments provided information of the students understanding throughout lessons. Formative assessments provided evidence of whether students continued with the unit pacing guide or whether additional time needed to be spent on certain content areas. The formative assessments where closely observed gave me an understanding of which students needed one on one time as well as who surprised other students. Pre-assessments showed students

prior knowledge to the content moving towards. During assessments is the most beneficial assessment because it shows evidence of whether re-teaching needs to be taken place or if the students are ready to continue. Post assessments provide information if the students gained the overall idea of the lesson. Formative assessments where beneficial towards the unit, without the formative assessments students are subject to be left behind in a content area. Pre-Assessment: Check list of Vocabulary, multiplication, and rounding formative assessment. The students will receive a score out of 4 to determine their prior knowledge of each section on the preassessment. Additional Unit Pre-Assessments: Morning work, Entrance Slip, KWL Chart, Class discussions, During Assessment (Formative Assessments): 1. Basic Facts Review- The students will complete a timed multiplication test. The students will be given five minutes to complete 100 basic multiplication facts. (The higher class did not have to take the timed multiplication test. During computer lab the students complete timed test weekly to practice skills and the students scores showed they did not need to complete additional time multiplication test.) 2. Book Work- Two days before the students took their mid unit test they were asked to complete one page from their books. The formative assessment allowed me to determine what to spend a large amount of time on during the test review. 3. Mid Unit Test- The students mid unit test allowed the information from the chapter to be broken apart. This will help the students focus less content at one time. The mid unit test focuses on multiplication properties, real world word problems, multiplication, estimating and vocabulary. 4. Quiz- The students will complete the multiplication two-digit by two-digit numbers as an additional grade as well as additional practice before the summative assessment. Additional Unit During Assessments: Whiteboard check, group discussion, class discussion, thumbs up thumbs down, graphic organizers, math journal checks. Post Assessment: Summative- The students will complete a end of unit assessment which will review vocabulary, multiplication, and real world word problems. Additional Unit Post Assessments: KWL charts, exit slips, quizzes, summative assessments. F55rg

Accommodations:
Extra Time Hard copy of direct instruction notes, Power Points, Smartboard activities (Student number one) Enlarged test (Students number one) Class Discussion Peer Discussion Re-teach

Adaptations:
After numerous testing situations student number one began receiving larger test. Instead of a test that included three or four pages his began to extend to seven or eight pages in length. As shown below the students work continued to run over into other testing questions. This affected his ability to read and understand the question being asked.

Student Three (High Level Student)

Unit Progress:
The students progress throughout the unit showed that the student was already at the top of the grading scale and did not have much room for improvement. The students grades maintained an A average. The student showed no signs of struggle throughout the unit and progressed at a pace that was acceptable for the high level group.

Post Assessment

Student Two (Middle Level Student)

Unit progress:
The students progress throughout the unit showed that the student had room for improvement. The students grade maintained an A to B average. When noticing the students grades were lowering the student was moved near a student that held good discussions and maintained an A average. After moving student number two and allowing him to participate in peer discussion the students grades improved throughout the unit.

Post Assessment

Student One (Low Level Student)

Unit progress:
The students progress throughout the unit showed that he maintained a steady B to C grade average. The student was placed in the lower level group to allow for more one on one time and accommodations. After viewing the students work and accommodating him during lessons (providing hard copies of materials) his grades improved. The student required accommodations that allowed him have more visual control of the material presented.
