July 2011 to present Support Linker Disability Services South East Region Beenleigh Service Centre Duties included: Assist the individual with a disability to utilise informal, mainstream and specialist disability services to achieve their goals, thus reflecting the vision of Disability Services - a society that values people with a disability, people with a mental illness and people who are ageing. Using collaborative practice and a person centred approach, discuss the outcomes of functional needs assessments, including the clients goals, and the recommended services and priority ranking for these services with people with a disability and/or families and work with the person with the disability to plan how they will access the services recommended by an Assessor. Build network and relationships with local communities, non-funded organisations and funded service providers to facilitate linking of clients. Creatively identify informal and mainstream supports that may assist each individual with a disability to access the recommended services. Once informal and mainstream supports have been fully utilised, facilitate matching between individuals with a disability and funded service providers where prioritised for a funded service response. Liaise with potential service providers to establish the availability of service. Make a formal offer of a service to the individual with a disability or their carer and complete relevant documentation. When required, support individuals with complex disability, communication or cultural needs to access and maintain engagement with the disability service system. Periodically conduct review of support needs of individuals with a disability, and the supports provided to them. When required, provide focussed short term responses to individuals in crisis situations.

Provide information and support to people with disabilities and their networks to facilitate the development of appropriate and flexible services and informal and formal supports. Develop and maintain knowledge of local services, including formal, informal generic and disability specific services and identify any gaps in available services and the need for the development of new services. Work with people with disabilities and their families to identify the cost of supports required and where necessary, be available to assist the individual and the family in their negotiations with service providers. Respond to changing needs of people over time and where necessary assist with renegotiating levels of funding and the required service responses. Maintain accurate records in relation to individual plans and monitor peoples satisfaction with the supports being provided. Manage funding processes in accordance with the public sector financial accountability.

Coordinate and implement mechanisms to ensure funding programs are delivered at a Regional level in an efficient and effective way. Provide ongoing information to people with a disability and families that have registered a need for funded support. Provide executive assistance and coordinate administrative support to Regional assessment, prioritisation and moderation processes. Manage information systems and related information and communication requirements in relation to funding programs. Participate in regional planning, monitoring and evaluation processes to improve and simplify needs identification, assessment, prioritisation and funding allocation processes including meeting regional targets. Oversight preparation of Funding Schedules and funding approval processes and liaise with relevant regional and central office staff to develop and implement funding arrangements.
