Mr. Andrew Ottens SRA Name_______________________________ Unit: Middle School Algebra, 6-8 grade.

Clear Purpose: The purpose of this summative/formative assessment is to evaluate students knowledge and understanding of some of the state standards associated with number sense and numeration. The following selective response assessment is a tool for both teachers and students, so that they can identify the learning targets that students may need help with. The twenty questions are chosen to reflect ten learning targets which come from four of the high school content expectations. If a student answers incorrectly to at least one of the questions linked to a particular learning target, then that student will be address and a re-teaching method will be found. Students can rate their confidence level as sure or unsure for each question and use that to reflect on their overall confidence with each learning target. While this formative assessment will not directly affect their grade, it should improve their mastery through self-reflection and possibly re-teaching. Mr. Andrew Ottens SRA Directions: The following formative assessment will be used to see if you have mastered certain learning targets set by the state. This is more to help see where you are in the class, rather than grade you. You will have 20 questions, each worth five points. It will include 6 True/False questions, 8 Fill-in-the-blank questions, and 6 multiple choice questions. If you were not a hundred percent sure on a question, please put a question mark next to the question number. If you have any questions during the test please raise your hand silently and I will come to you. Once you have completed all 20 questions to the best of your ability, please flip the test over and either draw pictures on the back or do anything that will keep you silent while the rest of the class finishes. There is no time limit for this test, so take your time and show me what you know. Good Luck!
