a. name
b. e-mail
c. phone number
d. message
e. button text submit/send
3. configure email address to get the contact us form
4. configure Email Subject when receiving the contact us form
5. configure Auto response e-mail text
6. configure successes message after submit
7. configure Error message for unexpected errors
8. configure if return to homepage after fill in the form or stay in the same page
9. Add free text area for users to fill in their comments
