Common Lodgement Errors for online MCV Applications
Incorrectly entered names and dates of birth
Please take care when entering names and date of birth and make sure you enter them as per the passport details. If these are entered incorrectly then it will either delay the processing of the application or may delay entry in to Australia for the crew member.
If an applicant only has one name then enter this in the family name field and leave the given name field blank. Do not enter FNU, not applicable or Nil etc as this will slow down the application process while we confirm the correct name.

Incorrectly entered passport country
Many applications are delayed when the country of passport is not entered correctly from the drop down list. Also, there is a question following the passport details asking if the applicant is a citizen of the country of passport. It is rare for crew to not be citizens of their passport country so in most circumstances the answer to this question is yes.
Incorrectly entered third-party details
Often third party details (eg shipping agent details) are entered incorrectly meaning that if contacted we can not provide information to the third-party as they have not been authorised on the application.
Incorrectly answered declarations
Health and Character Often incorrect responses are entered when answering the health and character declaration questions. These are yes/no responses so take care when answering that the appropriate option is chosen. Incorrect responses here can again delay applications while our department unnecessarily investigates applicants health or character.
Correct Information The last question on form asks applicants to declare that all information provided is true and correct. Often applicants click no instead of yes to this questions which again delays their applications while case officers seek to confirm if the information is correct.
