1. Verify that your name appears in the first record in the _SAMProjects table.
2. Create a new table. In Datasheet view for the new table, rename the default primary key ID field to TherapistID. Change the data type of the TherapistID field to Text. 11
3. Add the following five fields to the new table in the order shown; all of them have Text data types except HireDate, which has a Date & Time data type: FirstName, LastName, Certification, Affiliation, and HireDate. Save the table as Therapist. (Note: table "Therapist" not found; table "TherapistID" used as best-guess.) Table name should be "Therapist", but is "TherapistID".
4. Enter the records shown in the workbook into the Therapist table.
5. Hannah created a database named RehabCenter that contains a Trainer-Therapist table with data for additional new physical therapists. The Therapist table you created has the same design as the Trainer-Therapist table Hannah created. Copy all the records from the Trainer-Therapist table in the "support_Emerge_A10_CP1a_RehabCenter.accdb" database and paste them at the end of the Therapist table in the current database. (The file "support_Emerge_A10_CP1a_RehabCenter.accdb" is available from the SAM Web site.)
6. Resize the width of the Affiliation column so the width value is 20.5, and then save the Therapist table and close it. 11
7. Use the Simple Query Wizard to create a query that includes the FirstName, LastName, Certification, and HireDate fields (in that order) from the Therapist table. Name the query TherapistHireDate, and then close the query. Should return records from the "Therapist" table. Invalid query. 11
8. Use the Form tool to create a form for the Therapist table. Save the form as TherapistEntryForm, and then close the form. Should be based on the Therapist table.
9. Use the Report tool to create a report based on the Therapist table. Save the report as TherapistListReport, and then close the report.
