WHAT SHOULD I DO WHEN?


A. In the event of accidents/injuries: Attach copies of any relevant documents.
-

What are the schools injury policies? (e.g. What should you do if a child gets
injured?)
o Contact Ms. Mendez, assess first-aid, contact parents, fill out injury
form
What documents must you complete?
o There is an injury form to fill out.
Where is the first aid kit?
o The storage shed where all equipment is stored.
Who should you contact in the event of an injury?
o Ms. Mendez

B. In the event of fire/evacuations: Attach copies of any relevant documents.

What are the schools fire procedures?


o Evacuate towards field away from buildings.
Where are the fire exits?
o The fields away from the buildings by the playground and the front of
the school are the fire exits.
Where is the meeting point?
o The field away from the buildings.

C. In the event of unauthorized access of children: Attach copies of any relevant


documents.

What are the schools procedures should an unauthorized individual attempt to


access the children?
o Contact Ms. Mendez or other personnel through walkie-talkie or in
person.

D. In the event of school lockdown: Attach copies of any relevant documents.

What are the schools procedures should a school lockdown be enacted?


o Run to the nearest building with an open door, turn the lights off, have
everyone keep their heads down and hiding in the corners and try to
keep the kids calm.

ABOUT THE ACTIVITY SPACE:


A. Risk assessment of activity space: Identify all potential hazards in the
OUTDOOR activity space (draw a map of the area and indicated all potential
hazards).
-The many potential hazards in the outdoor activity space have been paved. In
the past, uneven pavement posed a threat to the students. Uneven dirt on the
grass field and falling tree branches are the only potential threats in the
outdoor activity space.
B. Risk assessment of activity space: Identify all potential hazards in the
INDOOR activity space (draw a map of the area and indicated all potential
hazards).
-The only potential hazard in the indoor activity space are tables which
students can run into and the slippery floor which students can slip on
especially if it is rainy outside and water gets dragged in the cafeteria onto the
floor.
C. Where are the closest restrooms and water fountains?
-Inside the cafeteria and near the basketball courts are the closest
restrooms. Outside of the restrooms and beside the portables are the
nearest water fountains.

ABOUT MY STUDENTS:
A. Risk assessment of students medical needs:
- Identify all students with specialized medical needs.
o Joseph Miranda- ADHD
o Elijah Martinez- dehydration from prolonged exposure to sun
o Francisco Jimenez- seasonal allergies
o Jenika DeWitt- wears glasses
- Be aware of student profile/medication and identify school medical
personnel responsible for these students

PRE-PROGRAM PREPARATION:

A. Where is the equipment located and how will I access it?


-Equipment is located in the storage shed outside on the blacktop and
there is a key to unlock it stored in the cafeteria sign-in cart.
B. Where will I go if it rains?
-If it rains, Tiger PRIDE goes inside the cafeteria.
C. How will I make sure the students are adequately protected from the
weather (sun/rain/wind etc)?
-To make sure that the students are protected from adverse weather
conditions, the cafeteria is available.
D. What will I do if a student is not appropriately dressed for Physical
Education?
-The student will remain with the group leader if he or she is not
appropriately dressed for Physical Education.
E. Where can I leave my personal belongings?
-Personal belongings can be stored in the shed for the duration of the
lesson.
F. What access, if any, do I have to the teachers lounge/facilities?
-There is a teachers lounge and it has separate restrooms and access to
cold water fountains.
