Please use the following steps:

o Go to this link: http://www.sefmd.org/
o Take cursor to the left side of the screen and click on Entry Form

o It will give you options, Click on On-line Entry
o It will take you to the Electronic Submission Page
o Scroll down and press on the link that reads as follows:
To register on-line now (entryformpart1), click here
o It will take you to another page, look for the part of the page that
has a box that reads: Select School name from list
o Click in this box and scroll down to find Crescent Academy
International, click on the name
o Go to the bottom of the page and press the SUBMIT button
o It will take you to the Official Electronic Submission Form
o Teacher Info:
Choose Duha Fahmy or Lena Abedrabbo as their science
teacher
o Student Information label
Describe to students what to plug in. All is required.
o You are not required to fill in the parent cell phone info.
o Junior Division
You have to choose the category that meets their
experiment needs (they should know which one it is).
o Senior Division
You will not fill in anything
o Team Project
You will say NO and skip the part for plugging in the
names.
o Section Two
SRC/ IRB Forms
Humans: any type of human being tested
Non-human vertebrate animals: any type of animal
that has a vertebrate
Potentially hazardous biological material: mold and
bacteria
Hazardous material: chemicals that need adult
supervision when being used
If you are unsure, they can go under General Info on
the left hand side of the screen, press forms and
refer to the forms they are talking about to see if it
includes them.
o Project has not entered any other competition, PRESS NO
o Explain Project Information
Title of Board: Include subtitle as well
By separating it with a colon
