    Private Medical Insurance Declaration For Approved Consultant

    All consultants working on Client projects MUST be in possession of a valid and adequate Private Medical
    Insurance (PMI) policy which should also provide cover for repatriation to home country and where
    required, emergency evacuation insurance before attending the Clients work location.
    If you do not currently have a PMI, repatriation and if applicable emergency evacuation policy in place you
    will need to obtain these before signing and returning this declaration. Proof of insurances must be
    provided at the request of CommsResources and failure to comply with such a request can invalidate your
    contract.
    If you have a valid and adequate private medical, repatriation and emergency evacuation (if applicable)
    Insurance policy in place, please sign the declaration below.
