Permission to sign-out equipment

Dear Parent/Guardian:

sign-out school equipment to complete various assignments. While the
equipment is being used by your student, they are assuming full
responsibility for it throughout the duration of time that they have it
signed out. If the equipment is lost or damaged, the fee listed below will
be due by check made payable the City of Peabody in order for your
student to receive a grade for the class.
