10/16/14 PASS- this week you had the opportunity to work as a CNA on the floor. You passed
of your vital signs as well as completed oral, cardiovascular, and skin assessments. You also
helped with ADLs successfully. You worked well the fellow students, staf, and had a patient go

10-30-14 Good day, did not get to see your head to toe assessment but you did well with
keeping me informed as to the progress of your patient. Be careful when using abbreviations, if
they are not accepted universally they cannot be used (see care plan page 1). Great job on
collecting your assessment data. Under medication in the last column you want to put the most
important thing a nurse should assess before giving that medication.. Your care plan was not
to bad, be specific in your interventional teaching and see comments for further clarification.
Under the evaluation of your interventions you want to assess if your interventions worked, if they
did we continue with those, if not then we change the intervention to get the measurable outcome we
want

You have a great bedside manner and you are always very respectful to the staf and clients. I
can see that you are getting a better grasp of the care plan with diagnosis, and interventions.
We still need to work on the medications efects and assessment. You will progress to 3 nrsg dx
over the next couple of weeks. With a UTI dx I usually think first of things like skin integrity,
because someone who is older may become incontinent due to the UTI, fluid balance, the
patient may be dehydrated d/t decreased fluid intake d/t pain. So just to give you some ideas as
you look at your diagnosis without information from the patient you can draw some conclusions.

This is the care plan that we went over in depth at the shot clinic, be sure to go over those
things we talked about and ask questions as needed to deepen that understanding. You are
doing a great job, in just 6 weeks time you are able to make some conclusions about your
patient and make some decisions regarding care. Good job. Medication administration is a
challenge in this setting, but with careful review and attention to triple checking, you can
accomplish safe administration and patient teaching, so remember adaptation is everything.
Catholic Church shot clinic. Slow but still full of time to absorb the clients we saw, the info that
needs to be given before and after the shot. Education surrounding blood pressures and general
health questions. We also had time to look at your care plan in depth. Thank you for you
reflection. Good job..

Very slow today at the nursing home with few admits. Good job on your care plan. You always
bring joy to your patients, and a smile to their face, good job!!!
