Student Self-Evaluation (midterm):

Course Objective

Evidence/Indicators: (The student

has ...)

Progress
Satisfactory

Demonstrated professional responsibility

and accountability to collaboratively plan,
implement and/or evaluate a health
promotion project relevant to the
community population served, using
elements of the course website such as the
learning system and regular,
punctual communication with preceptor
and faculty.

During my time in this clinical

setting, I believe I have exhibited
accountability and responsibility by
taking it upon myself to come up with
a topic of interest while working with
my preceptor and accounting for the
opinions of other members in the
agency. This topic is extremely
relevant to our target population as flu
season is fast approaching and hand
hygiene is a key element of health
promotion in schools.

Applied their knowledge to support

population assessment, collaborative and
evidence informed health promotion
project development, implementation and
process and outcome evaluation.

After settling on a topic, I have

gained my evidence by performing a
critical needs assessment gaining
knowledge from both my preceptor
and completing a windshield survey
of the surrounding area. The planning
stage of our project is well underway
by gaining the necessary resources
from the Health Unit and online
research to create a presentation that
is both fun and educational for the
target audience. This part of our
project will be implemented in the
upcoming weeks.

Demonstrated ethical respectful and

professional practice when working with
community organizations and the
population with whom they work.

Demonstrated knowledge of relational

practice in project development, respectful
of group process, leadership/followership,
and collaborative practice to meet the
needs and build capacity of the population

I believe I have acted very

professional in the workplace by
adhering to the dress code, wearing
my nametag and addressing each staff
member in an appropriate manner. I
also have remained respectful towards
staff and students by finding a quiet
place to work that wont distract the
students learning environment while
maintaining confidentiality with the
agency.
By working with a project partner, I
have learned to adapt my working
process to collaborate ideas with one
another. I have taken on both a
leadership role by establishing a

in the given context.

Demonstrated self-regulation through the

development of a project plan and
milestones, learning plan, and ability to
independently seek means to meet goals,
reflect, and respond professionally to
feedback to meet learning and project
goals that align with the SONs program
goals and the Community Health Nursing
Standards of Practice in Canada.

contact relationship with valuable

resources, such as the health unit, and
also taking a followership role by
accepting that my partners ideas may
be better than my own and working
together to achieve our goals together.
Self-regulation is a key aspect of my
clinical practice as we have the
opportunity to moderate our own
schedule so we can achieve our
weekly goals that will progress
towards our final outcome. Also since
the school day is only 6 hours long, I
must fulfill my 8-hour obligation by
going to the library to regulate my
own learning. Being given this much
freedom has allowed me to develop
skills that promote responsibility for
my own learning.
