Personal Statement: I am a passionate, goal-driven, creative individual. I have strong planning and organizational skills. I readily
adapt to change, work well within groups as well as independently, and have the ability to multitask without compromising quality of
performance. I have strong communication capabilities and a highly motivated outlook. I am a self-directed learner with exceptional
problem solving skills. I have experience working with a broad range of diagnoses, implementing interventions, and modalities to
restore function and mitigate disability. I have the ability to make interventions client centered and ensure collaboration with the client
through the OT processes. My experience is in treating pediatric, adult, and geriatric populations. I am skillful in managing many
patients treatment needs, organizing caseload demands, and completing documentation in an efficient and timely manner.
Mission Statement: My mission as an occupational therapist is to be creative, client-centered, skillful, ethical, compassionate, and
driven. I value continued education and active learning in my practice to meet the dynamic needs of the people I serve. I strive to
promote health, well-being, and participation in meaningful occupations throughout the lifespan. I will ensure the use of evidencebased research and innovative problem solving in my practice. I guarantee the promotion of diversity, collaboration, trust, and
positivity in the treatment of my clients.
Strengths: I am a hardworking, diligent, and disciplined individual. I work well with structure and routine, but have the understanding
of the importance of change and progress. I critically plan and problem solve before taking action or making decisions. I am respectful
of procedures and policies, and strive to adhere to the expectations set of me. I have the ability to overcome any obstacle with personal
determination and purpose. I am an ambitious, self-starter who anticipates a need and executes a plan independently. Lastly, I am
committed to stable values. I ensure delivery of all commitments and to meet all obligations showing trustworthiness and
dependability.
Growth Needs: Due to my driven personality I have a tendency to push myself to the limit. I often take on new roles and
responsibilities with the eagerness to help others, while unintentionally overwhelming myself. I have developed great organization and
multitasking skills to aid me in determining my ability and allowance to take on new tasks. Another area of growth is the expectations
I have for myself and others. I hold myself and others in my life to the same high standard. Therefore, I expect others to take
ownership for their word, work, and self-improvement which does not always happen. I strive ensure a personal understanding of the
strengths and weaknesses of others in my life, so that I can align myself to maximize our performance together.
Leadership Model and/or Style Selected: I selected the leadership style of transformational leadership because I have the ability to
inspire others to improve upon their expectations, perceptions, and motivations toward their goals. I am a firm believer of leading by
example and that includes all aspects of myself (personality, traits, and abilities). I value trust, respect, and fairness among all people
