Long Term Goal:

The client will increase expressive language to improve her ability to


communicate wants, needs, and request skills by increasing vocabulary and utilization of
gestures, when given a purpose to communicate.

Short Term Goals:
1. The client will engage in appropriate turn-taking skills by attending to peers turn
and waiting for own turn with 60% accuracy with moderate cueing.
2. The client will count and recognize numerals during facilitative play with 70%
accuracy with minimal cueing.
3. The client will identify basic colors during facilitative play with 80% accuracy
with independent cueing.
4. While in view of preferred items placed in target locations, the client will use a
single or set of prepositions and verbs to describe the location of the items, in
order to obtain access with 60% accuracy with moderate cueing.
5. The client will verbally imitate a 2-word utterance with 60% accuracy with
moderate cueing.

Procedures:
1. The clinician and client begin the session playing with Jack-in-the-Box to
facilitate requesting and prepositions.
2. The clinician uses hand-over-hand to help client draw a butterfly and bear on a
piece of paper to help aid with colors and requesting.
3. The clinician takes down each star from the stackable star toy when the client
requests them.
4. The client plays and utilizes the pop-up Elmo for colors, numbers, and turn taking
with the clinician.
5. The clinician uses bubbles with the client for her to say prepositions and
requesting.

Subjective Data:
The client was pleasant and corporative. She is a preschooler (4-5 years old) and
they always want to play with something different. Her caregiver/mother mentioned she
has been doing well at home with communicating more.
Objective Data:
1. The client engaged in appropriate turn-taking skills by attending to peers turn and
waiting for own turn with 60% accuracy with moderate cueing.
Current Data: With cueing: 1/3- 33%
Without cueing: 0/3- 0%

2. The client counted and recognized numerals during facilitative play with 90%
accuracy with independent cueing.

Assessment:
The client was pleasant and corporative. The client is still having issues with two
out of the five goals. They are engaging in appropriate turn-taking skills by attending to
peers turn and waiting for own turn and verbally imitating a 2-word utterance with 60%
accuracy with moderate cueing. The other three goals are either at or almost reaching
100%. The two goals the client is having issues with will take a little time and needs to
have consistency. The client does very well with identifying colors, counting and
recognizing numerals, and with prepositions and verbs. The cueing for all of these also
varied.
Plan:
The clinician will continue on current goals. Also, the clinician will begin to fade
two of the goals. They are counting and recognizing numerals and identifying basic
colors during facilitative play. The client is doing very well with them. Then, the
clinician can begin to introduce more two- word utterance goals. The client will continue
to go step-by-step until the final goal, which is to increase her expressive language to
communicate her wants, needs, and request skills.
