TERMS AND CONDITIONS

1. I understand that once I certify with a client I will be issued a SOW (Statement of
Work/Contract) and I must meet the SOW requirements for the client in which I have
certified to service. I understand that the SOW is renewed every 3 months and that if I
wish to terminate the SOW, I need to contact Linking Loop via email at the end of the
SOW period.

I understand Arise and/ or Linking Loop have the right to terminate my

SOW/Contract at any time.

3. I understand that paydays will be on the 14 and the 28 . What I earn from the 1 th

15 will be direct deposited to my account on the 28 of the same month. What I earn

from the 16 to the 31 will be direct deposited on the 14 of the following month. I
understand that if my first check is less than $20, it may be held over and added to the
following payday.
4. I understand and agree to give a 30-day notice should I choose to terminate
my contract with Arise, and/or with
Linking Loop.
5. I am aware that Arise withholds a fee from each pay period in the amount of $19.95
(subject to change), which is paid to Arise. I understand that this fee is subject to go
down as the team grows.
6. I understand that I am an Independent Contractor and not an employee of Linking
Loop. Depending on the opportunity that I chose; I may be paid by the hour, by talk
minute or by the call. I understand there is no guarantee of a set hourly pay if I choose
an opportunity paid by the talk minute or by the call. Most opportunities offer
incentives and some also offer commission, which I will be paid for based on my
contract. I understand that I am responsible for reading and understanding the Client
Opportunity in its entirety.
7. I understand Linking Loop will be deducting 15% of my amount paid
from the client to me as their fees, including any bonuses and incentives.
8. I understand that Linking Loop will not deduct taxes from my check. I will receive a
1099 at the end of the year and understand that I will be responsible for paying my
own taxes. I understand that if my contact information should change, I am responsible
for updating my contact information in the Arise portal as well as notifying
Linking Loop via email.

9. I agree to hold Linking Loop harmless of any claims/lawsuits due to conduct/

negligence of clients, Arise or any other sub-contractor.
10. I understand that if I refer 5 people to Linking Loop and they are actively servicing their
client then, my American Choice fee of 15% will be lessened to 5%. I understand that if any of
my referrals should cease servicing their client, then my American Choice fee will go
back up to 15%.
11. I understand that my client certification class cost may be reimbursed to me by Linking Loop. I
understand that this reimbursement will be broken into payments and added to my
direct deposit. I understand that in order to qualify for this reimbursement, I must meet
the following criteria:

I must pay for the class

I must pass the class and certify with the client
I must meet or exceed my Service Level Requirements as determined by my
SOW for the entire SOW period.
I must request reimbursement and provide proof of payment to Linking Loop via email at the
end of my first SOW period.
I must be still working with Linking Loop and actively servicing my client.
