Apply best practices for release forms:
1. Make the release obvious
2. Educate and inform of risk clearly
3. Be specific
4. Obtain information to help establish understanding of
the risks
5. Encourage questions about the release
6. Give the participant time to review the release
7. Use key declarations
8. Release and assumption of risk section should come
right before signatures
9. Parents and minors must both sign a release
10.
Each participant must sign a separate release
