THE MANAGER
AFPMBAI Main Office
Col. Bonny Serrano Road corner E. De Los Santos Avenue
Quezon City
Dear Madame,
GREETINGS OF PEACE!
This letter is in response to your reply dated August 24, 2014 to my
membership termination request dated July 23, 2014 in which you stressed
among others that, the AFPMBAIs Premium or upgraded Basic Insurance
(UBI) is COMPULSORY/MANDATORY upon entry to active duty and as such,
it can only be surrendered or stopped upon separation from service. This
is not true. As a matter of fact, I became member of your organization not
because it is compulsory and mandatory but because you sent your agents
to us to offer membership.
And that there is nothing in the mandate of the BJMP that requires its
personnel to be compulsorily obligated to become a member of your
Association and that can only be terminated upon separation from service.
What is prevailing is our mandate to be an Automatic, Compulsory and
Mandatory member of our very own BJMPMBAI upon entry to active duty and
can only be withdrawn upon termination from service.
Further, a lot of the BJMP personnel are not even member of your Association
upon entry to service though some were, but had successfully terminated
their membership at will without any hassle.
Therefore, I would like to reiterate my demand for the immediate
cancellation of the AFPMBAIs deduction in my pay slip to wit; a) MBA
Addtl-P914.00 and B) MBAP-25.00 and the refund that I may receive from
my policy contribution less any legal deductions if any. It is my humble belief
that your organization will expectedly deal with my modest request without
unnecessary delay. Requirements for termination were already sent in the
previous request.
My right to become a member of the AFPMBAI includes my right not to
become a member.
Immediate approval will be highly appreciated.

Respectfully Yours,
Name
