ACP would like to continue providing School-Linked and Co-Located services within each
school district throughout Dakota County. As the attached data from the last five years of the
program demonstrates, ACP is not only able to make available School-Linked services to each of
the districts, but can also expand the referral based model within each district and/or school. In
addition, the next phase of the RFP allows for more expanded Co-Located services with
interested districts for identified schools (DHS has also targeted SED and District 917 for
expansion). Please see the below DHS guidelines for required letter of support:
Demonstrate the school district(s) commitment to support providing school-linked
and/or co-located mental health services. The school district(s) may describe plans
for strengthening connections between the school staff and mental health
professionals as well as the school's commitment and capacity for support and
resources, such as dedicated private space.
Letter(s) should be signed by the superintendent(s).
Letter(s) should reflect an understanding that the Responder will be negotiating a
contract or memorandum of understanding with the school(s) to specify levels of
commitment, terms for coordinating and integrating services within the school(s)
such as referral processes, in-services for cross-training, participation to attend
team meetings, and confidentiality concerns.
In addition, ACP is in agreement with and, if funded, is able to provide ongoing School-Linked
and
Co-Located services, as outlined by Janelle Schilman in her RFP informational sheet submitted
at the last Collaborative meeting (05/31/13):
Reporting of bi-annual outcome data to collaborative board and shared with
designated school district
Bi-annual sharing of Family Satisfaction survey results including return rate
Clear communication tools regarding referral process, referral source contact and
client contact expectations (i.e. provider will contact referral source within 5
working days and client within 3 days)
Providers and the schools to develop triage systems to define students who may
benefit from this level of intervention
Provider to participate in care coordination conferences, IEP or 504 meetings
and/or provide feedback to the referral source as to the status of the service
provided to the student and family (ancillary services)

Clear communication as to who the schools can contact when they have an issue
or question regarding the program or mental health practitioner assigned to a
student
Closing summary sent to referral source as outcome of service for each student
including whether referrals for additional mental health services were completed
If multiple providers are selected, a bi-annual provider meeting facilitated by the
Collaborative to review and assess current system of referral and processes,
address barriers created by districts, the county or individual schools (different
requirements, expectations, background checks), and discuss state of financial
feasibility
