REZUMAT
Evaluarea aspectului i esteticii dento-faciale este dificil, ntruct acestea sunt subiectiv cuantificabile, percepia medicului i pacientului putnd fi diferit. Faa reprezint partea cea mai expresiv a corpului uman,
fiind un factor determinant n integrarea socio-profesional. Pierderea dinilor afecteaz aspectul facial general i conduce frecvent la traume psihologice, alternd totodat funcia fonatorie i masticatorie. Prin
urmare, este esenial ca lucrrile protetice s furnizeze un rezultat favorabil din punct de vedere estetic i
funcional. Obinerea unor restaurri protetice funcionale i integrate natural n estetica feei reprezint o
provocare constant pentru medicul dentist.
Scopul acestei lucrri a fost identificarea principalilor factori implicai n analiza estetic a zonei frontale maxilare, investignd relaiile dintre dinii frontali maxilari i diverse structuri i repere faciale. n acest sens a fost
analizat literatura de specialitate utiliznd urmtoarele baze de date: Medline, ScienceDirect, Wiley Online
Library. Criteriile de includere pentru selectarea materialelor au fost: articole publicate n reviste cu factor de
impact, articole al cror coninut a fost accesat full-text, articole relevante pentru tema aleas, articole i cri
scrise n limba englez sau romn sau al cror text a putut fi accesat ntr-una dintre aceste dou limbi.

ABSTRACT
Assessment of dento-facial appearance and esthetics is difficult as they are subjectively quantified, a patients and
doctors perception being often different. The face is the most expressive part of the human body and a key factor
in the socio-professional integration. Losing teeth affects the overall facial appearance and frequently results in
psychological trauma, altering mastication and phonetics. It is therefore essential that prosthetic treatment provides a favorable esthetic and functional outcome. Achieving functional and naturally integrated prosthodontic
restorations is a constant challenge for the dentist.
The purpose of this paper was to identify the main factors involved in the esthetic analysis of the maxillary anterior region, investigating the relationship between maxillary anterior teeth and different facial features and landmarks. In this regard, a literature review was conducted using the following databases: Medline, ScienceDirect,
Wiley Online Library. The inclusion criteria were: articles that were published in journals with impact factor, full-text
articles, relevant articles to the chosen topic, articles and books written in English or Romanian or whose text could
be accessed in one of these two languages.

PROPORIA DE AUR
n literatura de specialitate se menioneaz faptul c cel mai armonios raport dintre dini este dat
de proporia divin sau raportul de aur (5,7-11).
Acest raport are valoarea de 1:1.618, iar semnificaia
estetic a acestuia a fost descris pentru prima dat
n Grecia antic (9). Numrul 1.618 este reprezentat
prin simbolul i este denumit phi, provenind
de la numele sculptorului grec Phidias (11). Raportul de aur a fost definit ca fiind cel mai atractiv
ochiului i minii umane, putnd fi gsit att n art
i natur, ct i n medicin, astronomie sau alte
tiine (10). Faa trebuie s fie n conformitate cu
proporia divin pentru ca aspectul facial general s
fie considerat unul estetic. De exemplu, dac distana dintre punctele antropometrice Alare (Al) este
1, atunci distana dintre punctele Cheilion (distana
intercomisural) este 1.618 (Fig. 3a). De asemenea,
dac distana dintre punctele antropometrice Al
(Alare) i Me (Menton) este 1, distana dintre Alare
i Trichion (Tr) este 1.618; iar dac distana dintre
Cheilion (Ch) i Menton (Me) este 1, atunci distana
dintre punctele Exocanthion (Ex) i Cheilion este
1.618 (5,10) (Fig. 3b).
La nivelul zonei frontale maxilare, dinii
anteriori privii din norm frontal trebuie s
ndeplineasc urmtoarele condiii conform regulii
de aur: incisivul centralul superior trebuie s aib o
lime de 1.618 ori mai mare dect a incisivului
lateral superior, incisivul lateral superior trebuie s
aib o lime de 1.618 ori mai mare dect poriunea

PROFILUL FACIAL
Profilul facial poate fi drept, convex sau concav,
evideniind normalitatea sau diferite anomalii (12,
13). Un profil convex st la baza unui retrognatism
mandibular, prognatism maxilar sau ambelor anomalii, n timp ce profilul concav se traduce printr-un
prognatism mandibular, retrognatism maxilar sau
ambele (12).

n analiza unui zmbet estetic, din punct de vedere al profilului facial vor fi luate n considerare
urmtoarele repere: unghiul nazo-labial, unghiul
mento-labial, linia estetic Ricketts (5,6,14) (Fig.
5). Unghiul nazo-labial se msoar ntre columela
nazal i buza superioar, are o valoare de 85-120
i depinde de poziia antero-posterioar a incisivilor maxilari, poziia vrfului nasului n plan vertical i de morfologia buzei superioare (12). Valori
mai mici indic un maxilar proeminent i un profil
convex, n timp ce valori mai mari se traduc printr-un
maxilar retrudat i un profil concav (6,13). Unghiul
mento-labial se formeaz ntre buza inferioar i
menton. Acesta depinde de nclinarea incisivilor
inferiori i de nlimea facial anterioar, avnd o
valoare medie de 110-130 (12). ntr-un studiu
realizat de Owens et al (14) pe 253 de subieci din
ase grupuri rasiale diferite, unghiul mento-labial a
fost singurul reper statistic semnificativ pentru toate
cele ase rase investigate, fiind considerat astfel un
parametru de referin n analiza estetic a profilului
facial. Linia estetic Ricketts trece prin vrful nasului i brbie. Se msoar distana dintre aceast
linie i buze. n mod normal, buza superioar se afl
la o distan de 4 mm de linia estetic Ricketts, iar
buza inferioar este situat la o distan de 2 mm de
aceasta (6,12).

Pedrosa et al (27) au descoperit o corelaie semnificativ ntre forma feei i forma incisivului central maxilar. La subiecii cu faa oval i ptrat,
forma predominant a incisivului central maxilar a
fost cea oval, n timp ce subiecii cu faa triunghiular au prezentat o form ptrat a dinilor n
60% dintre cazuri. Astfel, se poate afirma c forma
oval a fost cel mai frecvent ntlnit la ambele
sexe. Forma triunghiular a fost cea mai puin do-

CONCLUZII
Medicii dentiti posed o capacitate relativ mare
de a recunoate i a aprecia la un pacient dentat
acele valori fizice majore ale dinilor i feei care
contribuie la aspectul estetic i caracterul personal
individual. Se pare c o astfel de abilitate nu este la
fel de bine dezvoltat atunci cnd este nevoie de
recunoaterea la un pacient edentat a acestor relaii
care indic forma i dimensiunea dintelui artificial
necesare pentru a restabili aspectul facial i funcia
fizionomic. Acest deficit este cauzat, n parte, de
faptul c faa pacientului edentat sufer modificri
fizice i funcionale considerabile, astfel nct vizualizarea valorilor estetice normale este ngreunat
semnificativ.
Este foarte important ca medicul dentist s cunoasc i s neleag dorinele estetice ale pacientului nainte de nceperea tratamentului, innd
cont i de faptul c pacienii de sex feminin iau n
considerare mai multe repere faciale i dentare pentru aprecierea unui zmbet estetic. Astfel, printr-un
tratament individualizat cresc semnificativ ansele
de a obine un rezultat final optim din punct de vedere estetic i funcional.
Printre factorii eseniali implicai n analiza estetic a zonei frontale maxilare se numr forma
feei, profilul facial, dimensiunea i forma dinilor
frontali maxilari.
