Student Strengths:
Allison is a friendly student that has many friends and interests outside
of school. She plays softball and goes to the gym. She loves to shop
and go to the movies with friends. Allison has good oral expression and
interpersonal skills. She consistently gets Bs and Cs in her high school
classes. Allison enjoys volunteering in her sisters school classroom.
Student Needs:
Allison has a specific learning disability in reading comprehension and
written expression. Allison also struggles with her organizational skills
and has trouble managing her time. Allison turns her assignments in
late and struggles to use her daily planner to track her homework. Her
parents are worried about her time management skills and that she
may struggle during college.

3. Follow up with student and parents to ensure they are reviewing
information or if they have any questions. Set up meeting date
for December to discuss student plans in detail.

1. Allison will volunteer her time to help with the kindergarten
round up for the local school district. This will increase her
contact with local teachers and professionals that she can learn
from.
2. Allison will contact the local YMCA and sign up to volunteer once
per week.
3. Allison will participate in her youth summer camp program
fundraiser with her youth group.

Plan to develop workplace and/or vocational and/or


educational experiences
1. Allison will work in the elementary kindergarten classroom during
her senior year of HS directly helping students and engaging in
hands-on activities with students.
2. Allison will observe a pre-school or kindergarten teacher for 3 full
days during her senior year and write a 1 page reflection of her
experience.
3. Allison will work with special education teacher and school
counselor to ensure appropriate classes are taken to complete
her senior course work to enter college.
Plan to develop community connections to services.
1. Allison will continue to participate in her community softball
league.

Allison has her drivers license and can transport herself to and from
volunteer experiences, work and school.
