VIP TABLE RESERVATION REQUEST

This form is used to request and reserve a VIP seating area inside EVE LOUNGE, located inside The
B,O,B" downtown Grand Rapids, MI,

2, and will include admittance for up to JS. guests (fllinimum purchase applies).
3, Cover charge does not apply, Your group will be admitted to The B,O,B, and EVE free of charge,
4. Your reserved area will be designated by velvet rope and your group will receive tableside service.
5, Specific seating requests we will attempt to accommodate, but cannot guarantee.

This reservation is for bottle service only All bottles include all mixers (excluding red bull or bottled water).

Eve opens at 11 ,00pm. This reservation will require a credit card number, which will be retained and no
deposit will be taken. Your reservation will be held until 11:30 pm, at which time if the group has not
arrived, you will be charged a $100,00 no show fee to the credit card provided and your reservation
forfeited, Upon arrival, before 11:30pm, you may select an alternate form of payment, or choose to have
your bill charged to the credit card on file (a 20% gratuity will be added for your server), All attendees will
receive instant admission to the nightclub with no wait in line,
