AUTHORIZATION LETTER
To whom it may concern, I am Noel Mayor a subscriber of your
company. I wrote this letter to authorize my son Neil Mayor to change
our DSL service from our current plan to that of Bundle Plan 1299 .
I apologize that I will not be able to present myself due to the reason that
I am currently living in our provincial house far from where the subject
DSL service is connected.
I hereby allow Neil Mayor to sign any documents regarding this request
on behalf. I have no objection on this authorization.
Rest assured that I take full responsibility for this authorization.
Attested to this letter are my IDs for identification purpose. I hope for
your kind consideration.
