Introduction
Population is defined as: The total number of persons inhabiting a country, city, or any
district or area (Dictionary, 2015). The focus of this paper is the analysis of a population that
resides in the 95351 zip code area of Modesto, California.
Profile
Demographics
The 95351 zip code in Modesto, California has a population of approximately 46,458
individuals. The population density is 5,238 individuals per square mile. Housing units consist
of 13,749 total units. Of those units, 34 percent are owned outright, 11 percent are owners with a
mortgage, 45 percent are renters, and 10 percent of the units are vacant. Of these households, 54
percent have children under the age of 18 years old (United States Zip Codes, 2014). Racial
makeup of this area consists of, 68 percent Hispanic, 16 percent Asian, 12 percent Caucasian,
and 3 percent multi-racial (City-data, 2015). The child population of 95351 as of 2012 was
16,073 individuals. Life expectancy in the 95351 is 78.55 years compared to the US average of
78.8 years of age (Stanislaus County Community Health Status Report, 2013, p. 180).
What about other demographics such as population age distribution, occupational distribution,

Morbidity
Serious patterns of illness in the 95351 zip code include asthma, obesity, mental health
illness?, substance abuse and suicide. Given its Central Valley proximity with plenty of farm
land and pollution from industry and automobiles asthma is quite prevalent. Low income
families and a lack of health education have created a population, as most of the United States,

which is obese. Mental health issues many times go hand in hand with substance abuse and at
times suicide. Underserved areas, such as the 95351 area, often lack essential services and
access for the population to effectively manage mental health and substance abuse issues

Stanislaus County Health Services Agency (SCHSA) in 2013 conducted county wide data
on illness patterns that led residents to seek care in the Emergency Department (ED) for the time
period of 2006-2010. SCHSA used age adjusted rates for each zip code in Stanislaus County so
a fair distribution could be made. The 95351 ED visit rates for hypertension included 240.9
individuals with a hospitalization rate of 82.3. The mortality rate for essential hypertension and
hypertensive disease was 17.1, the highest in Stanislaus County. ED visits for ischemic heart
disease was 49.0, the lowest rate in the county. The age adjusted hospitalization rate was 465.2.
This gave 95351 a mortality rate from diseases of the heart at 178.3, the second lowest in the
county. ED visits for cancer came in second highest by the 95351 at 33.0. Hospitalization rates
also third highest at 341.3. Cancer mortality rates for 2008-2012 were 137.8, the lowest in the
county. ED visits for diabetes were 485.2, the highest by far in the county. Admission rates of
281.1, again by far the county highest. With diabetes mortality rate of 29.6 which is the second
highest county wide. ED visits for asthma were the highest at 326.6 county wide with a
hospitalization rate of 64.9, also a county high. Alzheimers mortality rate was lowest in 95351,
probably due to its low average resident age. On the other hand, the 95351 was highest in
chronic liver disease mortality at 21.6. Chronic lower respiratory disease mortality rates were
third highest at 54.1. The 95351 topped the homicide rate at 18.0. Mortality rate from nephritis,

Nephrotic Syndrome & nephrosis was second highest at 12.8. Parkinsons disease mortality was
second lowest at 4.2. Pneumonia and influenza was in the middle with a mortality rate of 17.6.
Rate of suicide was second lowest at 6.8. Unintentional injury topped the county at 53.5
mortality rate (Stanislaus County Community Health Status Report, 2013, p. 143-178).
Birth Rate
The 95351 zip code has a seven percent birth rate with and 64 percent of the mMothers
are unwed. How does this rate compare to other zip codes or Modesto as a whole or even
county? Of these, 53 percent of the unwed are on public assistance. Unwed Mothers age 15 to
19 years make up 18 percent, ages 20 to 34 make up 67 percent, while ages 35-50 make up 15
percent. Unwed mothers by race included 67 percent being Caucasian, 88 percent Black, 50
percent were Hispanic and Asian mothers were at 100 percent (Towncharts, 2013).
Mortality Rate
Major causes of death in the 95351 zip code include heart disease which leads and
accounts for 31 percent of deaths. Next is cancer with accounts for 24 percent. Chronic lower
respiratory disease makes up for 7.4 percent of deaths. Unintentional injury is 6.4 percent of
deaths. Alzheimers at 5.5 percent and diabetes mellitus accounts for 3.1 percent of deaths in the
95351 area (Stanislaus County Community Health Status Report, 2013, p. 167). Note that ages

Socioeconomic Characteristics
Income varied greatly in the 95351 area. There were 1135 households reported an
income of less than 10 thousand dollars yearly, 1232 households reported a yearly income of 10
to 15 thousand dollars, 1229 households reported 15 to 20 thousand dollars yearly income, 1230
households at 20 to 25 thousand dollars yearly income, 740 households at 25 to 30 thousand

dollars yearly, 703 households with reported income of 30 to 35 thousand dollars a year, 780
households with 35 to 40 thousand dollars yearly, 668 households with 40 to 45 thousand dollar

thousand dollars yearly, 1064 households with reported income of 60 to 75 thousand dollars

Residents with some college were at 10.3 percent, with 1.7 percent attaining an associates
degree from a junior college, 2 percent of the population held Bachelors degree, and 1.4 percent
held college graduate degrees (Intero Real Estate, 2009).
The workforce in the 95351 zip code consisted of a 9.7 percent unemployment rate as of
June 2015 (Zip Data Maps, 2015). White collar workers consist of 4677 residents but note that
many mom and pop stores exist in this area. Blue collar workers consist of 2394 individuals
(Intero Real Estate, 2009). This leaves a large room for children and individuals on Public
Assistance.

Housing affordability ranges from a studio at approximately 594 dollars per month to a
four bedroom home at 1,609 dollars monthly for rent. Housing stability has an average of 4 year
median residency. The annual resident turnover rate is 20 percent. Five year residency was 30
percent (Intero Real Estate, 2009).
Eighty-four percent of unwed mothers and their children live below the poverty level is
this nationally? (Towncharts, 2013). As of 2015 the U.S poverty rate is 11,770 dollar per year
for one individual, family of two is 15,930 dollars and 20,090 dollars for a household of three
annually (U.S. Department of Health, 2015). With that being said, over 3000 households have a
stated income fewer than 20 thousand dollars annually.
Psychological Climate
Self-Concept
The population of zip code 95351 is very diverse and on an individual level have their
own concept of what the community is like. When driving around the area, the perception is
different than those who live and work in the community. One of the key informants that was
willing to answer questions was Tamara (personal communication, September 16, 2015), the
security guard with Maxx Value Foods on Paradise Road. She had been working in this area for
at least 5 years, and lives within this community. When she moved here she was wary of the
population from what she had heard from others outside of Modesto. She says that now working
in this public position, she has been in contact with many of the transients and addicts that
normally people do not want to meet. There are many homeless people, drug addicts and
transient people within this community that make it a scary place for many people. Tamara stated
they are people too, just down on their luck and some are even trying to get better. It was
interesting to see how people who actually get time to talk with the homeless, like Tamara, view

the community as opposed to those like the surveyors who just merely see it at a glance. She also
had stated that being on a personal level with them allowed her to see what they were capable of
doing in order to better themselves. One of the former drug addicts is now on methadone to
abstain from heroin use, he now washes windows for change outside in the parking lot. He is
harmless and patiently stands and waits outside of the Walgreens Pharmacy next door to the
Maxx Value Foods only offering his services to those who accept. It all depends on the individual
outlook of the person as to what the area looks like. Some see 95351 as rough neighborhood and
others see it as a mix of good and bad.
Attitude Toward Health
The attitude toward health is lacking in this area on a person to person basis. There are
facilities at the publics disposal however it is up to them to use them. There is a great clinic off
of Paradise Road that offers many healthcare options to the public, if they are willing to take use
of them. The clinical manager Irma Miguel (personal communication, September 16, 2015) was
very helpful in explaining all that is available to the public at their facility ranging from bBirth to
gGeriatrics. They even have a psychiatrist available on Mondays. At the clinic, no one is turned
away. They have a satellite Supplemental Security Income (SSI) office next door and are able to
obtain access for medical aid right away. The people need to be aware of these services when in
need of care in order for these facilities to be of use. Tamara (personal communication,
September 16, 2015) did tell a story about a couple who were both heroin addicts and together
are now supporting one another to get on methadone at the local clinic so they can live a better
life for themselves. Tamara also told us a story about a homeless man that got clean, and then
relapsed and overdosed behind the Maxx Value Foods. There are many homeless and addicts still

in the area who dont wish to change themselves or who fall prey to the same unhealthy habits of
drugs, fast food, and no exercise.
Historical Changes
Born of the need for expansion of the railroad through the great Central Valley of
California, Modesto began its existence with populations from smaller towns along the
Tuolumne River - Paradise and Tuolumne City. Their populations not only moved lock, stock
and barrel to the new village laid out by the Central Pacific Railroad, they carted their buildings
along, too. The dusty village soon became the center of Stanislaus County, dethroning Knight's
Ferry by becoming the county's sixth seat of government since 1854 (Historic Modesto, 2015).
Per Frank Johnson (personal communication, September 9th, 2015) as the city grew many
people moved from the downtown area to the newer growing areas, leaving the 95351 area to the
lower income community. They built public housing and allowed refugees and lower income
families live in these areas for subsidies to no cost.
Nutritional Evaluations
Sources of food
There are very few large grocery outlets in this district. Many of the locations are liquor
stores that were converted to have food or groceries in them. These locations do not have the
highest quality food or produce that is required for adequate nutrition. Gas stations and
convenience stores specialize more in junk foods and easy to grab food over those that have
nutritional value. The grocery store, Maxx Value Foods, is the main source for produce and is
located towards the west side of the district on Paradise Road. There are plenty of fresh groceries
and produce available here, the public in the store all seemed in a rush and no one was
approachable to ask if the store was adequate in providing their needs. The foods and variety

present in groceries did appear to be plenty and the prices were moderate. However, the amount
of time the individual spent shopping depended on if they lived close and if they had children
with them or not, from our observations. The rest of the region consists of fast foods locations
and local ethnic restaurants such as Lao cuisine near Fifth and H Street.
Nutritional Statistics
Based on information from Healthy City (2009) the average life expectancy in the zip
code is between 72 to 78 years of age, with some highlighted zones having individuals live from
80 to 81 years of age. This can be due to a number of reasons, however on the map view the
areas with the highlights tended to be closer to grocery stores and more toward the farm lands
than in the rural city area. This can mean that those who may have access to healthier food
options tend to make healthier choices. The death rate value from Health City (2012) was 5.21 to
6.889 based on total number of deaths per 1000 residents and based on the other zip codes in
Modesto which was significantly lower. The statistics show that the area is not starving but could
use more health promotion and nutritional education in order to make healthier choices.
Physical Fitness
Fitness facilities
There are not many gyms or recreational facilities within this region. The main ones that
were found were the Community Centers for seniors, Maddux Youth Center, SOS Health and
Social Club, Neighbor Hood Center at Marshall Park and the many other scattered parks. The
major gyms are all located outside of the area code, but there are two locations within the district
that are main recreation centers, Maddux Youth Center and SOS Health and Social Club. The
Maddux Youth Center is a non-profit youth center that provides a safe environment for all ages
to be able to enjoy recreational activities such as basketball, weight training and even boxing

classes. The center also offers classes for GED training and English classes for those non-English
speakers. The staff there are known as specialists or counselors, Antoinette was one of the staff
specialists and was able to explain the rules and services of the center. The requirements to use
the facility include filling out an application to the center, having an ID card, and leaving all
types of gang affiliations at the door. Antoinette (personal communication, September 16, 2015)
stated that, This is a neutral place where kids and adults come to relax and socialize in a healthy
way; you would not know that any of them were gang members. Some of them take off all gang
related stuff before even entering the facility. For the adults they also have a job placement
program, as well as help them apply for SSI and Cal Fresh at the facility as well. This was a great
resource for those who cannot afford to drive out of the district or cannot afford to pay
membership fees like at SOS Health and Social Club.
The latter of the two available resources for those in this region included the SOS Health
and Social club. This facility has many physical and social activities available to their
memberships based on a monthly fee. There are multiple packages available and based on the
amount of facilities they wish to partake in the club. They offer basketball, handball, tennis,
volleyball, group exercise classes, golf and swimming (individual or team). There are also halls
to be used for events. A family could sign up, which includes children under the age of 15 and
have access to the clubs many facilities. The fee ranges based on age and can be bought
individually, as a couple, or family package. Example, a young adult age 20 who would like
individual access to the club would have to pay $50 a month. The first and last month dues plus
an initiation processing fee of $100 is required at time of joining. The minimum time of contract
is one year, so the young adult would have to pay $200 in order to join the club, with a 50%
discount if youre a new member for the first year. So even though they provide many services,

someone that has a limited income or survives on SSI would not be able to afford this club (SOS,
2015).
Attitudes of Residents
The residents of this area have plenty of resources to be physically active with parks and
Community Centers, however not many take advantage of them. How do you know this? The
activity level can also be based on the predominant age within this zip code. The population of
seniors versus those younger is significantly lower. According to American Fact Finder (2013),
the largest population in this district is the age category between 10-14 years of age being 10.2
percent of the total population. There are significantly higher populations of adolescents to
young adults in the area. The 65 and older only comprise roughly eight percent of the population.
The older population has the smallest representation within this area. This can mean a number of
things in terms of fitness and health. The young tend to be more active in terms of energy versus
those who are aged that prefer a more sedentary lifestyle. However, just because they are
younger and have more energy, does not mean they are healthy or know they can use the
facilities in the zip code to their advantage. This section focuses on attitudes, but I dont see
attitudes addressed
Physical Examination
Systematic observation of community
This observation consists of the West and South area of Modesto in the 95351 zip code.
There were no new or recent developments noted in this area. Homes and buildings appeared
older and majority was noted with security doors, windows and/or gates. Very few
neighborhoods were noted with sidewalks and poorly paved streets. Most of the homes appeared
well maintained on the exterior and some were noted with makeshift windows made out of large
wood boards on other streets. There were many fast food restaurants and convenient stores

located throughout this area. These convenient stores were barred with metal barriers on the
windows and doors and noted to be a high traffic area with customers continuously entering and
exiting. There was one particular grocery store called Maxx Value Foods that appeared to be in a
convenient and closest location to people living in this area. Maxx Value Foods was noted with
only one entrance and exit to the store possibly as a deterrent to theft. Parks looked well
maintained but transients/homeless lay around in their makeshift shelters or beds and group of
teens were huddled around during school hours. Stray dogs were frequently seen roaming the
streets and dogs were also noted almost in every yard as if it were a deterrent to unwanted
visitors. There were many graffiti noted on commercial buildings that appeared artistic and some
that may possibly be gang related with unique letterings. There were small old churches noted
that blended in with other homes in the neighborhood. There were two recreational community
centers noted next to parks. There were a couple of pharmacies both small mom and pop ones
and chain pharmacies noted in close proximity to homes. Homeless and unkempt individuals
were frequently seen wandering neighborhood streets and commercial areas. The schools in this
area were noted in close proximity to homes and appeared like a safe ground for the students.
Metal fences fenced in the perimeters of the schools and campus securities were noted walking
around the school perimeter. On the perimeter of the west and south side of the 95351 area well
arranged grape vineyards, strawberry fields and orchard trees were seen.
Defined parameters/boundaries
The 95351 zip code area appears like a triangular map that consists of where North
Carpenter and North 9th street intersect then heading south from North Carpenter Road to the end
of South Carpenter Road. From South Carpenter Road this zip code extends to the end of East
Hatch Road. From East Hatch Road along the Tuolumne River extending northwest towards
North 9th street completes the last route through the 95351 area code (Maptechnica, 2015).

Climate
Modesto usually has mild winters and very hot, dry summers. Average January
temperatures range from 56 F (13 C) in the day to 40 F (4 C) at night. Most of the rainfall
occurs during the winter and the annual total is 13.2 inches (33.5 cm). Since the city does not
have a full storm sewer system, many streets flood during winter rain storms. Average July
temperatures range from 95 F (35 C) in the day to 63 F (17 C) at night. During the summer
months there can be multiple days in a row with high temperatures exceeding 100 F (38 C).
This can pose health risks for people with weak constitutions or who ignore the dangers of heat
stroke. Onshore breezes (locally known as the "delta breeze") moderate these high temperatures
somewhat, with cooler air coming in after 8 or 9 PM on summer nights (U.S. Climate Data,
2015).
Location
The 95351 zip code is located in Stanislaus County in the urban city of Modesto,

Environmental conditions
General description- the 95351 area is pretty well-maintained, no apparent littering or trash was
noted on streets or neighborhoods. Sidewalks lacked in some of the neighborhoods and people
were seen walking on dirt roads or on the street. Limited storm drains were seen on area streets,
which may cause concern for flooding during rainy season.
Housing quality and condition- there were many homes and public housing in this area and

generally they all look in good structural and livable conditions. Really?
Degree of pollution
There are 27 local facilities that contribute to toxic release of pollution into the land, water and
air. Modesto, CA ranks 830 out of 893 urban areas in the country based on total pollution
released per square mile. Top five chemicals released to the land and air are nitrate compounds
(89 percent land, 30 percent air), cobalt compounds (six percent), glycol ethers (two percent land
and 18 percent air), ammonia (two percent), xylene (one percent land, 13 percent air), styrene

noted as modes of transportation during this community observation.
Health System
Private/Public Services
The area in Modesto California with the zip code 95351 does not have any hospitals.
There are three main hospitals in the greater Modesto area which are Doctors Medical Center,
Memorial Medical Center, and Kaiser Permanente. Those living within the 95351 zip code have
a limited availability of health care options to choose from. There is a clinic on Paradise that has
a community agency within it, so people living in the area with no insurance can apply on site
for Medical. There are non long term care facilities found in the zip code, but Modesto has over
ten long term care facilities, such as Acacia Park and Casa de Modesto. There are a few home
health agencies serving the Modesto area. Addus Healthcare, Gentiva Health Services, Global
Healthcare, Interim Healthcare, Sutter care at Home and Tenet homecare of Doctors medical

Center. The Valley Family Medicine Residency Program, provides resident physicians the
opportunity to do their clinical hours at Doctors Medical Center and at several Stanislaus
County health service agencies and outpatient facilities. This program allows the resident
physicians to treat patients in the community which ultimately adds more physicians available to

There are several primary care doctor offices throughout Modesto however, not all accept
medical or accept new patients. Golden Valley Health Centers are stationed throughout Modesto
how many? and offer a wide variety of services including teen health, womens health, senior
health, pediatrics, health education and behavior health services.source?
Other Healthcare or Related Resources
School nurses play an important role in the community in Modesto. During the 20122013 school year, there were 16 nurses employed in the school system to serve over 30,000
children and their families.how do you know this? School nurses provide health assessments,
medical and mental health and social referrals. There are a few occupational health services
with the main one being, Sutter Gould Occupational Health Services, which offers a variety of
services from pre-employment drug screenings to work related injury exams. Stanislaus County
in a partnership with the United Way has a disaster preparedness referral program. In the event
of an emergency people in the community can dial 2-1-1 and receive referrals for hundreds of
resources in the community. The Modesto Fire Department offers classes and training for their
Community Emergency Response Team (CERT) program. This program empowers residents to
help themselves in the event of an emergency and their family member and neighbors through
basic first aid and cardiopulmonary resuscitation and other skills such as knowing how to turn

Volunteer agencies in the Modesto area the Salvation Army and Modesto Gospel
Mission which both provide food and shelter to homeless. Seniors living in the community also
have a variety of resources available such as dial a ride, fitness classes, fall prevention, family
caregiver support program and meals on wheels. Catholic Charities has a senior program for
people aged 55 years or older where people in the community can volunteer to spend time with
elders for social interaction in elders homes or in a care facility. Elder helpers in Modesto also
provide opportunities for elders to have volunteers spend time with them doing fun activities to
help prevent social isolation and despair.
Per the Stanislaus County Health department website, the public health department has
walk in or appointment based immunization clinics and a wide variety of services for the
community. The Public health department also offers sexually transmitted disease testing, HIV
testing on a walk in basis. Appointments can be made for immunizations, TB skin testing and
immunizations for travel. The Adolescent Family Life Program and Cal-Learn is home visit
program that serves pregnant teens and parenting teens and provides support with classes,
education and encouragement for these young people to finish their high school education. The
teens can receive services up to 19 years of age and are seen by a case manager who is also a
public health nurse. Healthy Birth outcomes is a program that supports high risk teens who are
pregnant through health assessments, a comprehensive care plan and assistance with access to
prenatal care. The teens in this program are followed by a public health nurse, a social worker
and a case manager. Another program offered in Modesto is California Childrens Services is a
statewide public health service that arranges and pays for medical care and equipment for
children and young adults under the age of twenty- one who have an eligible medical condition.
The public health department has a program for medical marijuana identification card, women

infants and children food assistance programs and nutrition and obesity prevention program.
People living in the community can find available resources for substance abuse and
recovery services. There are services for drug rehabilitation that are court ordered and other
services that are voluntary. Genesis Narcotic Replacement Treatment provides a twenty one day
program and has helped many homeless in the area who want to get off of heroin and become
clean of drugs. Stan works behavioral health services offers assessments and counseling for
people who have problems with mental health, substance abuse and even domestic violence.
There are countless resources within the Stanislaus County for people of all ages. There
are programs for alcohol and drug use, teen pregnancy and teen parenting and the elderly.
People living within the community will most likely qualify for some of the services offered in
the county, however, not all people may know about the different types of programs available.
People in the community need to be educated about the different community services available
and then hopefully become empowered to utilize the many resources offered in the county.

Review of Non Health Systems
Political System
The city of Modesto is governed as a council-manager system. They mayor is elected by
the entire city. The six members of the city council are elected from districts by the voters within
the respective districts (modestogov.com). The area of the 95351 is split into two districts,
District 2 and District 3. The current mayor of Modesto is Garrad Marsh. The District 2 council
member is Tony Madrigal and the District 3 council member is Dave Lopez. The city council
meets in formal session on the first, second, and forth Tuesday of each month at 5:30 pm 1010
Tenth Street. All city council meetings are open to the public, except portion of meetings in

were in shopping centers. The current issues that some of our informants mention in the area
were the hHomeless population and the lack of shelters and food for them. Drug abuse was also
mentioned. Many people in this are abuse meth and heroin per our key informants. Stashing
there syringes in the parks, where children play, in trees, behind signs, etc.incomplete sentence
Social System
The prevalent norm in most of the 95351 area that was observed by my group was
poverty. Many people in this area live below the poverty line. Many of the buildings in this
neighbor hoods are run down, old and lack sidewalks in the majority of the housing area.
Per Tamara (personal communication, September 16, 2015)indicated that most of these
residents consist of multigenerational families living in the same home, i.e. grandparent, children
and grandchildren. She also stated that this area is predominantly Hispanic.

Nirvana Drug and Alcohol Treatment Center focuses on substance abuse treatment
services. It offers sliding fee scale and facility payment assistance to help cover the cost of
treatment (addicts.org)
Maddux Youth Center adjacent to Cesar E. Chavez Park, is a newly remodeled center that
includes a full court basketball court, gym, weight room, community room, reading room,
computer lab and game room. They center offers a variety of non-fee sports and fitness activities
for youths of all ages, tutorial and learning opportunities and a variety of special events

Parent Resource Center (PRC) mission is to build stronger, healthier families by offering
education, mentoring, and supportive services to empower and strengthen our community
(prcfamilies.org) The PRC offers support to families through volunteer mentoring and on site
parenting education classes to decrease the risk of child abuse.
King-Kennedy Center located at 601 Martin Luther King Drive offers ongoing programs
for seniors, community resource and youth activities (modestogov.com)avoid one sentence

The Redwood Family Center-Corson site houses 25 women and their children who would
otherwise be homeless and lack the access to necessary recovery resource in Stanislaus County
(valleyrecoeveryreasources.org)
There are many parks in the area of 95351, Bellenita Park, John Thurman Field, James
Marshall Park, Mark Twain Park, Mellis Park, West Side Park, Paradise Park, Robertson Road
Park, and Mancini Park (city-data.com). While we were assessing the area many of these parks
had poor playground equipment for children to play in and many of them were well kept and but

had lots of graffiti. Another thing noted was lots of Hhomeless people were sleeping, drinking
alcohol and using drugs in the play area for children.
Communication Patterns
People can find out about what is going on in the area by many ways. Word of mouth is a
very informal means of communication. Many, schools in the area have websites that keep
parents up to date on information that is going on in the school and upcoming event. They can
also read about what is going on in the Modesto Bee, which is the local newspaper. With the
popularity of social media many people can find out what is going on by way of Facebook news
pages which include, centralvalleytv, The Modesto Bee, Modesto News and Modesto Police
Department. Source?
There are not that many billboards that were seen in the 95351 area. But the few that
were seen were in English and Spanish and contained things information regarding health.
Education System
The zip code of 95351 consists of two high schools, one public and one private. It has
one junior high school which is also public, and eight elementary schools, seven which are
public and one that is private.source? These schools and their demographics are shown in
appendix A thru E.
I spoke with a Modesto High School teacher, Mrs. Megan Ramsey (personal
communication, September 11, 2015). She said that Modesto High School has many sports
programs that are eligible to all students to participate in if they maintain academic eligibility.
This school also offers Agriculture programs, International Baccalaureate Program (IB) and the
Performing Art Academy. The Agriculture Program teaches students about agriculture science,
such as mechanics, anatomy and biology of an animal and horticulture. The IB Program is a

rigorous academic program that if completed and tested out can count for college credits. The
Performing art Academy is a first of its kind in the area. It is a course that covers all aspects of
the performing arts, including scenery, costuming, make up, sound, lighting, business, theatre
management, dance, drama, vocal performances and photography. Mrs. Ramsey also stated that
Modesto High School offers and Infant Care program for students that have children that attend
the high school. They also have a special Education department that helps to educate disabled
students. Modesto High School has an 88.0% average graduation rate which is above the national
average of 82.2% (K12.niche.com)
The Modesto City School district offers a pregnant minor program. This program is a
voluntary program that student can be placed into the semester that they are pregnant. It is
designed to meet the needs of pregnant teens. It covers the academic requirements to help them
earn a diploma. This program also offers courses in prenatal care and parenting skills. It is
provided at two locations, Downey High School and Elliot Continuation School (nces.ed.gov)
There is not a library that is found in the 95351 zip code, but you can find the city Library
in downtown Modesto.
Economic System
The major businesses in the 95351 area per city-data.com are A&B Truck Repair, Gilroy
Foods, Equilon Enterprise, Modesto City Schools, Modesto Irrigation District, Foster Dairy
Farms Incorporated, JM Equipment, and Macco Auto Painting.
Many of the shopping centers in this zip code are on the out skirts of this area. The major
shopping centers in the 95351 zip code per city-data.com are the Ceres Shopping Center, Ceres
Plaza Shopping Center, Caruso Shopping Center, and Modesto Marketplace Shopping Center.
There is also a Maxx Value Food that can be found on Paradise Ave near the heart of the 95351,

but not much else really exist in this shopping center. The Ceres Shopping Center has K-mart,
Home Depot, and a Rancho San Miguel Super Market. The Ceres Plaza Shopping Center has a
few different restaurants, a Factory 2 U, Dollar Store, Big 5 Sporting Goods Cost U Less Foods,
and Payless Shoes. The Modesto Marketplace Chopping Center has a few restaurants, Harbor
Freight, Cost U Less Foods. The Modesto Marketplace is the nicest of the four shopping
Centers. There are not many places in this area to shop for clothing and other house hold goods.
For the population 25 years old and above in the 95351 zip code per city-data.com 56.8%
of the population has a high school degree or higher, 7.3% have a Bachelors Degree or high,
1.9% have a Graduate or professional degree and 25.2 % of the population in 95351 are
unemployed.

Risk for Powerlessness related to lack of role models, following in the family cycle, and feeling

Subjective Data
Ideally, this section would be a synthesis of the communitys point of view related to the
problem
Population analysis team interviewed Tamara at Maxx Value Foods located at
Tuolumne and Paradise streets during the mid-afternoon on September 16th. Tamara was a
security guard (badge #1339) at the food store from 1pm until 11pm. Interviewee stated
residence in the 95351 for the last 5 years. Previously she had lived a sheltered life in the
boonies of French Camp, California raised by her Grandparents. Tamara was well in tune with
the patrons of the food store and nearby Walgreens customers alike. She stated community and
healthcare resources included, the Urgent Care Clinic (Paradise Clinic) just 50 yards from the
food store coupled with a built-in Community Service Center (CSC). A womens homeless
shelter was located just down the street named Redwoods in which her former partner was
now a resident of. It is also a place for women addicts that have been released from jail and need
a place to help stay clean. Recreation areas were noted as a youth center named the Maddux
Youth Center located right at the 95351 and 95354 zip code border. Tamara stated that despite
her good relationship with patrons of the food store and local drug addicts, she does not feel safe
in the area and maintains a constant state of vigilance. Biggest safety concern was from
transients, the homeless, addicts, gangsters, and the mentally ill individuals that frequently
inhabit the area. She was a victim of violent crime in which she received a serious Traumatic

Brain Injury (TBI) as a result of domestic violence. She surprisingly noted an increase in heroin
abuse. Tamara stated that the Maxx Value Food store supplied all grocery needs for the area.
She did not discuss nor care about politics. Being that she was not educated in the 95351 she had
no information on the schools locally. Tamara at one time did receive government assistance
while living in the 95351 but has no longer needed it since recovery from her TBI and full-time
employment. Tamara stays in touch with the vibe of the neighborhood view careful
observation along with word of mouth from the local population. When asked about gaps she
was quick to state that homelessness is a major problem that appears to be much more prevalent
in the 95351 area (Tamara 1339, personal communication, September 16, 2015).
With Tamaras guidance the population analysis team walked to the Paradise Clinic and
interviewed Irma the clinical supervisor who has been employed at the clinic for 15 years. Irma
stated that the clinic serves the community from birth to the senior years and essentially turns
nobody away if care is necessary. The facility is a two tier, a Family Practice office with a fullyfledged MD as well as a Psychiatrist on Mondays. The second tier is the clinic which is staffed
with medical residents, interns, and mid-level practitioners. The clinic only takes Medi-Cal,
Health Plan of San Joaquin (HPSJ), and cash patients. No private insurance is accepted due to
the Federal funding of the program. Irma stated most of the clients are of Hispanic culture but
African-American, Asian, and Caucasians also receive care. Access to specialty services,
although not available at the Paradise Clinic site, is readily available at other locations in
Modesto or the Bay Area if need be. Within the building is the CSA or Community Services
Agency which can quickly qualify those in need of medical services coverage and food money
alike (Irma Clinical Supervisor, personal communication, September 16, 2015).

Looking at the community from a nursing perspective, there are many aspects that need
to be addressed. The population cannot maintain a healthy standard of living if the infrastructure
is not up to par. The streets have no sidewalks in many neighborhoods and housing conditions
are rundown, they are minimal in their upkeep. Many housing units consists of multiple families,
in small rooms. There are bar fences surrounding homes and windows. The recreational areas for
children and adults are not safe. The parks are not clean and many have stray needles laying
about. Stray dogs are also very prevalent in the parks where some homeless reside. The
neighborhood community center is attached to the fire department and located next to Marshall
Park, the facility is under strict lockdown that in case of an emergency would take time to get
help. The more urban parts of town have slightly better conditions however streets are congested
and run down making driving conditions unsafe for both drivers and pedestrians. There are
handful of grocery stores within the zip code, with one being the main store and only a couple of
community centers. There are homeless in all areas of the zip, ranging from under bridges to
empty lots. While conducting our home visits in clinical it is apparent that poverty is
multigenerational. Generations of families all live in the same household and it seems that there
is little to no ambition to get out of their situation. Many children are following in their familys
footsteps. It appears that the children in this area are with little education options or are unaware
that they can attend college with little or no out of pocket expenses.
Assessment
Interpretation of Data
The 95351 area in an underserved community that falls below the federal poverty line.
The majority of its population are ethnic minorities. The community needs more accessible
grocery store, better housing and safer neighborhoods. The community feels isolated from the

rest of Modesto. The environment of the fire station was uninviting. The parks are overtaken by
?? inviting for young families to utilize. There are no hospitals in the area which prevents people
from getting the medical attention since many families do not have adequate transportation.
People in this area are more concerned with daily survival instead of making a meaningful
contribution to society. There are multiple schools throughout the area and they appear to be safe
for children. Schools are fenced and have security present on campus. There is a lack of
shopping not only for healthy foods, but also for clothing and general household goods. There
are a lot of community resources available but people are not aware of what is out there. The
take away we got from this assessment are that the people living in this zip code are not bad
people they are just financially destitute.
Planneeds to relate to dx
Short Term Goals and ObjectivesI cant tell which are your goals, your objectives and
then the actions
1. Develop a healthy food program with a farmers market to help promote healthier eating
choices. By connecting with local farmers we will get locally grown produce to give
away to low income families. Local farmers would receive tax write offs for their
donation to this program and recognition through a banner at the free farmers market.
2. Develop a sex education program for children ages 12-18, or grades 6-12th grade. We
will inform students about the benefits of that the local clinics. Many people do not
know about safe sex practices, or that they can receive free birth control and condoms.
All children would be eligible and do not need their parents permission. We will create a
pamphlet and presentation partnered with Planned Parenthood and the Paradise Clinic to
assist with educating these young adults in the schools. Education will be yearly from

grades 6-12th at the local schools in this area. This program will not be parent driven as
to not dismay the children from seeking services.
3. Educate the community on available resources located within the zip code of 95351. We
will have pamphlets with available resources that will be distributed door to door and sent
home with their children from school. Billboards placed in the community with a phone
number automated to assist people with finding available resources. These resource
pamphlets will be translated into multiple languages so that people living in the
community can know the available resources.
Long Term Goal and Objective
To improve education levels in the community through educational awareness. We will
start a program in the 9th grade (but also available to all other grades) that will inspire children to
explore post-high school educational opportunities. Children will learn through workshops once
a month how to apply for college, federal student aid, grants, scholarships application and
college prep workshops. Each month we will invite guest speakers from local colleges to discuss
with students regarding their education opportunities. This program will promote post high
school education thus improving the quality of the life of these kids, by continuing their
education. We will also bring in guest speakers to our workshops that have come from similar
situations of poverty and created a better life from themselves and their families. The children
will be followed through high school to ensure success of high school graduation. A mentor
program and support group will be set up for these kids so they can help encourage each other,
have a place to vent and post success stories.
