Registration System

I (representing the Marketing Department) had a discussion with Yan Ru (head of Admin
and Awards Department) on the 17/8/12 and we come up with this system. There will be
receipts and registration forms.
The Receipt
Obviously there will be a receipt issued to all those pay. The information on the receipt
will be the name of the student, Student ID and receipt number (which will already be
there).
The Registration Form
There will also be registration forms for all the students who will be coming. The
information required on the form would be:
a) Name of student
b) Student ID
c) Vegetarian or non-vegetarian
d) Contact Number
e) E-mail
f) Amount of payment
g) Date of payment
h) Signature of student
i) Receipt number (at the corner of the form-this act as reference)
This registration form is important for the Admin Department to key in the students
particulars in a database.
The receipt alone is insufficient because we might not have the space to take down all
the said information above on the receipt.
> Those who are on duty will have to attach the copy of the students receipts to their
respective forms. This step is crucial so that we can track down the students who
actually paid. In case of mishaps like the receipt or the form going missing, then we can
refer to the latter. Even if we lose both, there is still the database.
