FOREWORD

The Department of Health (DOH) Regional Office No.VII is committed in fulfilling its thrusts relative
to the implementation of [Aquino Health Agenda/ Universal Health Care/ Kalusugan Pangkalahatan].
One of these primary thrusts is the [exercise of technical authority/ provision of technical assistance]
in putting in place [good governance in health/ good governance practices] as one of the vital
components that the local health system requires.
We, as an organization, sees the impact of the fragmentation of local health services leading to the
deterioration of integrative approaches to health care delivery system and quality of local health
services. These services would include the management of the referral system, which by its very
nature requires good coordination and cooperation in the delivery of health services at all levels.
We recognize that for Central Visayas, this is a huge task for us considering the existence of various
social, political and geographical factors. Moreover, we cannot continue to work effectively in a
situation where there is a break in the chain of integration of the Region's health care delivery system.
It is the goal of the Regional Office to rationalize the distribution and capacities of different health
facilities within the Region to provide appropriate health services, so that the poorest families would
be able to access quality and affordable health care. Most of our poor in Central Visayas go to
government-run health facilities in their time of need and they deserve no less than the best care from
our public health facilities.
The publication of this CENTRAL VISAYAS HEALTH REFERRAL SYSTEM MANUAL is a huge step towards
improving our efficiency and effectiveness through the implementation of a health referral system
that addresses the gaps and challenges relative to the appropriate approach and management of
referrals within the Region.
This Manual proposes therefore, to serve as our official reference to guide our hospitals and other
health care facilities as we continue to strive to be the technical stewards in health development. The
health referral system described in this Manual shall also further enhance the operation and
functionality of the Inter-Local Health Zone (ILHZ) System of the Region.
This document, developed in collaboration with specialists, experts, mentors and users of health
referral systems, is for the benefit of all public health practitioners.
Profound and unwavering commitment is critical for us to truly achieve Kalusugan Pangkalahatan by
2016. Its pursuit is our common goal. Therefore, let us all work together in realizing this dream for all
Filipinos!

PREFACE
Cognizant to the changes in the society and environment and the ever increasing needs of the ever
increasing population, the effects to health and health service delivery, its effect to the poor, the
vulnerable and the underprivileged will be inevitable and needs to be addressed.
The Central Visayas Health Referral System (CVHRS) Manual as with the other Health Referral System
(HRS) manuals was developed to address the challenges in implementing a HRS specifically the
disparity in matching the patients needs and linking them to the appropriate health facility.
In addressing the gap in referring patients, this manual will be used to guide healthcare workers and
other key stakeholders on the proper approach and management of referring patients and to remind
them of the ethical standards, respect, courtesy and attitude toward patients, significant others,
stakeholders in the community and their co-health workers.
Also, to bridge the gap between the preventive and curative medical services, the 3 C`s, Coordination,
Cooperation and Communication was emphasized as essential in the implementation of an effective
Health Referral System which was highlighted in our activities and in this manual. During the conduct
of the several activities of the CVHRS, a fourth C was added, Commitment, which was generally agreed
that if there is no commitment among the stakeholders the HRS won`t work. A 5th C was again added
during our M&E, Collaboration, which was evident as the monitoring team and the different
stakeholders in the health facilities conferred on the operationalization and making functional of the
CVHRS.
An innovation in the CVHRS is the ReCo Guide in Referring Patients Checklist which was created as
a result of the experiences and review in the implementation of the HRS. This is a one page step by
step procedure that will guide the referring health facilities in the mechanics and protocol of referrals
emphasizing on the mandatory requirement of making a call. ReCo is Referral/Consultation where
both parties discuss and agree on the patients diagnosis, management and disposition (whether to
manage the patient at their facility or send the patient over).
To ensure the quality and efficiency of the referrals, the stakeholders were always reminded on the
Guiding Principle in referring patients:
To Refer the Right Patient to the Right Facility at the Right Time all the Time

An interesting summary of the consolidated Issues and Concerns in the implementation of the HRS of
the different health facilities in Northern Mindanao and Central Visayas is tabulated in this manual.
This was presented during the seminars and consultative meetings, each issue and concern was
discussed, together with their respective resolve, recommendations, and agreements.
The concept and function of the ILHZ has always been emphasized, defining its role in leveraging the
delivery of health services and programs thru coordination with their LCE`s and the health personnel
with an active Local Health Board.
This manual is intended for the use of all healthcare facilities from the Barangay Health Stations, Rural
Health Units, City Health Offices, Hospitals (Government and Private) and other health facilities and
stakeholders.
A directory of the health services and facilities available in the region and other key national health
facilities and related offices is provided in this manual to guide stakeholders to identify and determine

the appropriate service, facility and location to respond to the health needs of the clients. Basically,
the concept of this directory is to be able match and link a health need to a facility or office in Region 7
and beyond.
One of the major concerns in the implementation of a HRS is the issue on Medico-legal Cases and
Death which have been the source of conflict among healthcare providers. Despite the previous
efforts and existing policies and guidelines, interpretation and compliance have been diverse. A
workshop, solely for this purpose was done to create for each province policies and guidelines. As
usual, as expected, heated arguments and discussions ensued, but as civilized and mature
professionals each province agreed and was able to create the expected output of the workshop. A
regional agreement was reached that unresolved cases will be referred to and decided by the
Provincial Health Officer and his/her decision will be respected.
The format of this manual was patterned from the Department of Health 2002 Manual on Health
Referral System and the NMHRS manual 1st edition.
The Central Visayas Health Referral System as with any HRS is evolving and is imbued on feedback,
cooperation, encouragement and support. As quoted, No canned health referral system will work,
but we, as a team and partners, are determined and committed to continue developing to achieve our
objective in implementing an effective CVHRS.
Hand in hand the DOH RO7 and with all our partners in health service delivery who enthusiastically
embraced the principles and concepts of the health referral system will pursue our goals.
Our resolve, to achieve our goal, to make available and accessible thru the CVHRS, Quality Health
Services especially for the poor and the underprivileged is our inspiration for the development of this
manual.

GENERAL OBJECTIVE:
To implement a Health Referral System that will ensure quality patient referrals among health care
facillities in Central Visayas.

SPECIFIC OBJECTIVES:
The different health facilities will be able to:
1. Utilize a Health Referral System Manual that will serve as a guide among health care facilities.

INTRODUCTION AND BACKGROUND:


The Central Visayas Health Referral System
A cornerstone in the Health Care Delivery System is to be responsive to the health care needs of the
community providing the appropriate health services matched and linked to the right facility which is basically
the essence of the Health Referral System.
This is the challenge in implementing an effective Health Referral System that has been experienced for quite a
time specifically in public health and government health facilities due to the limited resources in terms of
infrastructure, equipment, medicine, supplies, and health human resource. In addition, the expected roles and
responsibilities of the stakeholders and the recurring issue of maintaining proper decorum has undermined
and compromised the conduct of activities of the health referral.
The devolution in 1991 played a major role in governance, and with the implementation of the Local
Government Code (LGC), broke the chain of integration of the Health Care Delivery System in the Philippines
wherein the Hospital (Curative) and Public Health (Preventive) services were administered independently.
Municipalities, cities, provinces and regions began operating separately, resulting to the further segregation of
the public health system. This fragmentation resulted to the deterioration of integrative approaches and
quality of Local Health Care Services, affecting largely the management of the Health Referral System which
requires good coordination, communication, and cooperation in the delivery of health care services at all
levels.
Several activities in the past, initiated by the DOH Regional Office 7 were done to implement the HRS but
apparently were not sustained as evidenced by our initial baseline assessment last year with regards to
functionality and effectiveness.
An initial baseline assessment on the HRS of the four provinces of Central Visayas (Region 7) were the
following:

HRS manual available in two provinces but not followed and other stakeholders are not aware
Mechanics/Protocol in referring patients not well defined
No standard/uniform referral and return form/slips
Entry in the referral logbooks needs improvement
Recording and documentation of referrals is deficient
Policies & Guidelines on Health Referral & on Medico Legal Cases & Death were either not available,
not followed, stakeholders unaware or needs refinement
Directory of health facilities and services not available or needs to be updated and broadened
Proper decorum is a common major issue and concern
Poor communication and coordination
Lack of Awareness of a HRS
Non-functioning Inter Local Health Zones, no local health board meetings.

In 2001, the National DOH HRS Manual identified as deterrents to the implementation of a functional health
referral systemthe following:
Inadequate policies and guidelines on the referral system
Poor accessibility due to geographical location
Inadequate health human resources
Inadequate logistics and technical support
Poor knowledge, attitudes, and skills among health providers

As stated in the DOH HRS Manual, strengthening the Health Referral System need to upgrade the Health Care
Facilitys quality of health services, optimize the use of available state of the art equipment, and enhance its
capabilities in local health planning, decision-making, and monitoring.
Thru the Health Facility Enhancement Program (HFEP) of the DOH, most if not all of the Government Health
Facilities, the Hospitals, Rural Health Units, City Health Offices, many Barangay Health Stations/Urban Health
Centers and others were upgraded in terms of infrastructure and medical equipments. This upgrading aimed
to help in providing their needs to comply with the DOH Licensing and Philhealth Accreditation to provide
standard quality health services. Also, that these facilities will be responsive to the health needs of their
catchment areas and decongest the tertiary end referral hospitals.
Presently there are already several manuals on Health Referral System (HRS) at different levels, National,
Provincial, Hospital and regional manuals including this Central Visayas Health Referral System (CVHRS)
Manual. A review of these manuals revealed that basically they have common objectives, rationales, principles
and concepts. Though they have differences in the implementation of the Health Referral System specific for
each facility or locality, the main goal of providing quality and efficient health service in referring patients is
the same.
The Department of Health Regional Office 7 has conducted a series of Seminar, Training, Workshop and
Consultative Meetings for the CVHRS in the last quarter of 2014 and in early 2015 to all the four provinces of
the region. An M&E was also done in the first quarter of 2015 to create awareness of the CVHRS and to assess
and sustain its implementation.
In all these activities, Vicente Sotto Memorial Medical Center (VSMMC) as the regional end referral tertiary
hospital with its experience and expertise was always available as partner to provide the general support and
assistance.
Participants of these activities were the Provincial Health Officers, Chiefs of Government Hospitals, Chief
Nurses, Administrative Officers, City and Municipal Health Officers, Public Health Nurses, Philhealth
representatives, DOH RO7 RLED and HFDS Staff and other stakeholders. The general objective was to
operationalize and make functional a health referral system in Central Visayas.
These activities covered the following topics/course content for the CVHRS: the Legal and Administrative basis,
a review of the present status of the HRS in each province, review on the basic principles and concepts of a
HRS, introduction of HRS Manuals, presentation of the DOH general policies and guidelines on Medico-legal
cases and Death, the workshop on the creation for each province policies and guidelines, presentation of the
different referral forms, logbook matrices and the monitoring tool.
One of the highlights of the CVHRS is the creation of the ReCo Guide in Referring Patients - Checklist which
is an operational guide for the referring and referred health facilities on the mechanics and protocol in the
management and approach of referred patients, matching the patients need to the right health facility. ReCo
stands for Referral Consultation, this was developed due to the previous experiences in referred hospitals or
facilities, a common inference that, had a prior call been made, and a discussion and consultation of the case
had been done, a lot of cases need not have been sent for referral because the referring facility can be advised
on the management and disposition of these patients thru phone whether it is necessary to send patient over
or manage at their facility with advise.
As agreed and emphasized, a Mandatory Call is necessary especially for hospital to hospital referral and those
emergency cases needing special care and attention with vital data and information that needs to be
transmitted.

Why make the call:

The patient will be thoroughly assessed for the purpose of referral


Referrals sent to other health facilities will be appropriate and properly screened because of prior
consultation and discussion
Referring facilities can be advised on the management and disposition of cases that can be treated
and managed at their level without sending patients for referral
Necessary preparations in the treatment and management of referred patients by the referring and
referral facilities can be made
Anticipated events and transport needs addressed
Savings on economic cost of unnecessary or inappropriate referrals and those managed thru phone
referrals

A major concern in the implementation of a HRS is the issue on Medico-legal Cases and Death which have
been the source of conflict among healthcare providers. Despite the previous efforts and existing policies and
guidelines, interpretation and compliance have been diverse. A workshop, solely for this purpose was done to
create for each province policies and guidelines. As usual, as expected, heated arguments and discussions
ensued, but as civilized and mature professionals each province agreed and was able to create the expected
output of the workshop.
A regional agreement was approved that unresolved cases will be referred to and decided by the Provincial
Health Officer and his decision will be respected.
Previous experiences in implementing programs and projects have taught us that if there is no follow-up or
review, this will fail. A Monitoring Tool has been developed which was based on the agreements and next
steps on what is expected of the health facility and personnel, their roles and responsibilities for a functioning
and effective CVHRS. Stressed during the visits that the purpose of the M&E was to assess the implementation
of the CVHRS and identify and address the issues and concerns in its implementation and not to criticize, point
finger or blame the facility or individual. Also, during the M&E the Plan Do Check Act method was applied and
urgent issues and concerns will be addressed and corrected immediately.
With the previous activities and experiences gathered mostly in Northern Mindanao and also in Central
Visayas in the implementation of the HRS, numerous Issues and Concerns were raised in its implementation. A
consolidated list will be presented in this manual, from the BHS, RHU`s, CHO`s, District and Provincial Hospitals
as well as the end referral hospitals together with the recommendations and resolve to address these different
issues and concerns, an output during the previous consultative meetings and HRS encounters.
Our modest experience in the various activities in the implementation of the HRS has allowed us to identify
and determine the Requisites in the Implementation of an Effective HRS which is as follows:

In pursuing the goals and objectives of an effective HRS, we have always emphasized the pivotal role of a
functioning ILHZ, the well defined roles and responsibilities of the members and a regularly convened Local
Health Board promoting the 4C`s, Coordination, Cooperation, Communication and Commitment even adding
the 5th C, Collaboration. The Local Health Board meetings are very crucial in addressing the multifold health
problems in the ILHZ and in the effective implementation of the different health programs, the CVHRS
nonetheless.
Our resolve to make available quality health services for the poor and the underprivileged is our inspiration for
the development of this manual and the implementation of an effective Central Visayas Health Referral
System.

I. THE HEALTH REFERRAL SYSTEM


A. DEFINITION:
Section 33 of the rules and regulations implementing the Local Government Code, defined a
functional referral system as one that ensures the continuity and complementation of health and
medical services. It involves all health facilities from the lowest to the highest level. These services
shall be comprehensive and shall encompass promotive, preventive, curative, and rehabilitative
care.
DOH HRS Manual - the set of activities undertaken by a health care provider or facility in response to
its inability to provide the necessary medical intervention to respond to a patients need, whether
real or perceived. It is a regular daily activity of linking a patient to a needed service.
To be effective and successful, the networking system should have the following preconditions:
Complete trust and mutual respect among the members and willingness to cooperate
with others on equal terms
Equal sharing of responsibilities and workload based on organizational or individual
capacities and mandates
Equal access to resources
Pragmatic and realistic approach to the implementation of network activities
Strong emphasis on local or horizontal mobilization.
B. TYPES OF REFERRAL
1. INTERNAL REFERRALS- are those which take place within the health facility and from one health
personnel to another.
Reasons for referral:
Opinion or suggestion
Co-management
Further management or specialty care
2. EXTERNAL REFERRAL refers to the referral of patients from one health facility to another
a. VERTICAL - patient referral may be from a lower to a higher level of health facility and vice versa,
based on the role and responsibility of each category of health facility
b. HORIZONTAL - patient referral is between like facilities in different catchment areas.
C. FRAMEWORK FOR HEALTH REFERRAL SYSTEM
The referral system shall operate within the framework of the Inter-Local Health Zone (ILHZ):
The Barangay Health Station/Urban Health Center, Rural Health Facility/City Health
Office and Primary Referral Hospital (Municipal Hospital) provides primary medical care
A Core Referral Hospital (District Hospital) provides secondary care.
In general, referral from a Health Center to higher levels should occur in the following situations:
When a patient needs expert advise
When a patient needs a technical examination that is not available at the Health Centers
When a patient requires a technical intervention that is beyond the capabilities of the
Health Center
When a patient requires in-patient care

D. REQUISITES FOR A FUNCTIONAL HEALTH REFERRAL SYSTEM


A well-functioning comprehensive two-way referral system shall have the following:

Defined functions and responsibilities (i.e., service mixes for each level of care)
Identified health service delivery outlets (public and private) and services provided
Agreed roles and responsibilities of key stakeholders
Agreed standard case management protocols (treatment protocols and guidelines)
Agreed referral policies, protocols, and administrative guidelines to support the health
referral system
System to supervise, monitor, and evaluate quality of care, referral practices, and
support mechanisms
Facilities and health workers capable in implementing the health referral system
Core referral hospital should have at least four major services: Medicine, Surgery,
Pediatrics, and Ob-gyn. It shall also have ancillary services (e.g., laboratory, x-ray).

II. OPERATIONALIZING THE HEALTH REFERRAL SYSTEM


A. Steps in Setting-Up a Referral System (Please refer to the details in the DOH Manual)
1. Organizing the Referral System
The Provincial Health Officer (PHO) should initiate the idea of setting up a referral system with technical
support from the DOH. For the referral system to work, it is assumed that an Inter-Local Health Zone (ILHZ) or
its equivalent is in place, and that the ILHZ board or committee has been organized.

B. Referral System Flow Chart
There are several factors that affect the flow of a health referral system. It depends on the geographical
location, competencies of health personnel, availability of supplies, health facility capability, and the customs
and practices of the people.
C. Referral Procedure (After proper assessment and communication)
1. Patients coming from a referral facility shall bring with them a referral slip / note containing relevant
information
2. Once the reason(s) for the referral has been addressed, the patient shall be referred
back with a corresponding return referral slip containing relevant information
3. All referrals shall be recorded in a separate logbook specifically for health referrals (required for all health
facilities), both by the referring and receiving health facilities.
This vital information is needed for policy formulation and improvement of the quality of services rendered.
4. Patients shall be referred after the following have been satisfied:

Thorough assessment of the patient (history, PE, diagnostics)


There is a need for referral based on the guidelines
Communication and agreement with the referral facility
Conduct and needs of transport identified
Referral slip and accompanying documents accomplished
Proper documentation

V. THE CENTRAL VISAYAS HEALTH REFERRAL SYSTEM (CVHRS)


POLICIES AND GUIDELINES
A. General Policies and Guidelines

The Central Visayas Health Referral System must be observed


Hospital and field health personnel are expected to maintain proper decorum at all times in relating with
patients, patients relatives, co-employees and other health service providers/stakeholders in the HRS
Supervisors shall orient and train all hospital and field health personnel in the operations of the
comprehensive referral system, in the context of local area health zone
Coordination and teamwork among all health providers shall serve as a common approach to attain goals
and objectives
Services to be rendered to a patient shall depend on the facilities, its capabilities, and manpower
resources
Referral system shall take into consideration the general welfare of the patient and the capabilities of
facilities within the system
Tasks at any level of health care facility shall be clearly defined, mutually understood, and reasonably
quantified. Actual performance shall also be evaluated regularly
All patients shall be attended to immediately upon arrival, giving preference to emergency cases / or
seriously ill patients
Clear, written, health referral policies and guidelines shall be available in all health facilities. Standard
referral forms must also be available at any given time
Essential drugs and medicines shall always be available in all health facilities

C. Procedural Guidelines

Services not currently available shall be accessed from the next level of care
Patients who have been referred must be sent back to originating facilities for follow-up and
disposition
Cluster barangays and municipal health care units refer patients to the core referral hospital of the ILHZ
where they belong, unless services are not available in that area
Patients may be transported to and from health facilities using a service ambulance or other means of
transportation. Ambulance fee must be determined by the ILHZ and charged based on the patients
capacity to pay
Communication system must be in place to facilitate the referral
In areas or ILHZ where there is no government hospital, networking with private hospital facilities with
available services shall be developed
Available services at each facility shall be determined and a MOA between the private and municipal and
provincial government should be undertaken
Continuous training and updating of capabilities of the health service providers shall be of utmost
consideration
A separate logbook shall be maintained for monitoring and evaluating records of all patients
Each level of health care unit shall have a list of essential equipment

Adequate staff, facilities and other resources that support the system should be considered. Referral shall be in the
context of Inter-local Health Zone.

E. General Policies on Medico-Legal Cases (DOH HRS Manual)

As a general rule, all MHOs shall act as medico-legal officers in their municipality in the absence of the
provincial medico-legal officer
All requests for medico-legal examinations must be accompanied by an official request from the police
authorities of the concerned municipality or barangay
Medico-legal requests not within the capability of the MHO concerned should be referred immediately to
the NBI together with corresponding reasons for referral
In cases where the MHO of the area concerned is out-of-town and after all efforts to locate him / her had
been exhausted, the MHO of the nearest municipality within the ILHZ must perform the requested
examination
All medico-legal cases shall be the responsibility of the MHOs, unless the patient would require the
services of the hospital for further evaluation and treatment. During weekends and holidays, the hospital
can attend to medico legal patients
Transport vehicle to fetch the MHO must be provided by the requesting parties concerned. If autopsy is
conducted in a private setting, the MHO should be escorted by a police officer
Medico-legal fees shall be paid to the MHO based on the rates provided by the Magna Carta for Public
Health Workers. This policy is, however, subject to the availability of funds and the usual accounting and
auditing rules and regulations
In some instances where there are no MHOs available in the area or ILHZ concerned, the Provincial Health
Officer may, upon prior notice, direct any government physician, preferably with expertise on the case, to
perform the required examination. This is, however, subject to the presentation of a certification from the
Office of the Local Chief Executive concerned that the MHO is not available
All other policies not included herein in relation to the above-mentioned subject matter shall be referred
to the Provincial Health Officer for evaluation and approval and subsequent inclusion in this general policy
guideline on referral of medico-legal cases.

F. Importance of Case Management Protocol - (Please refer to the details in the DOH Manual)
The referral decision of an individual clinician is the heart of the referral system. Such decisions are based on his
professional attributes, knowledge of the health care system, and personal style. However, advancements in
diagnostic technologies and therapeutic modalities demand that the physician keep up with these changes
This is the importance of case management / Clinical Practice Guidelines (CPGs) to all health care providers. The
use of protocols is an approach to encourage good practice in the area of health referrals.

VI. THE PATIENTS BILL OF RIGHTS

The patient has the right to every consideration of privacy

The patient has the right to expect that all communication and records pertaining to his/her
care will be treated as confidential by the hospital except in cases such as suspected abuse and
public health hazards when reporting is permitted or required by law

The patient has the right to review the records pertaining to his/her medical care and to have
information explained or interpreted

The patient has the right to expect that within the capacity and policies, a hospital will make
reasonable response to the request of a patient for appropriate and medically indicated care
and services

among the hospital, educational institutions, other health care providers, or payers that may

VIII. HOSPITAL REFERRAL IN MASS CASUALTY INCIDENTS


A Mass Casualty Incident (MCI) is a major concern in hospitals, particularly in the Emergency
Department. In this situation, which happens without warning, the usual day to day functional
capability is overwhelmed to the detriment of the health service that is provided.
Vehicular accidents, which are the most common man-made disaster, involving usually a passenger
bus, can generate influx of patients affecting the hospitals response in emergencies. A large number
of victims may be involved and sum up to even a hundred or more, but these victims have different
injuries and severity also differ. As such, the importance of triaging or sorting out the victims
according to their emergency need, management and disposition, providing the appropriate
transport and identifying the right facility is crucial.
Of great importance, even at the site of the accident, is to control and know which victims need only
first aid treatment and those that needs to be sent at once to a hospital. This is to avoid the common
scenario in disasters of chaos and disarray in Emergency Rooms that even non-emergency victims
that can be treated in primary care health facilities and their relatives, crowd the ER. Patient care is
affected, resources depleted, ER operations compromised and health personnel are overwhelmed
who are also attending to the other sick patients and emergencies.
The Health Referral System, if utilized properly, matching victims with the service classification and
correlated with the Universal Triage System, will minimize and control the anticipated problems
during MCI`s.
PROCEDURE
1. All Type I and Type II patients should be transported to a Level III Hospital for complete
diagnostic service and immediate or emergent treatment.
2. All Type III patients will be referred to a Level I Hospital, for cleaning, suturing of wounds and
Stress Debriefing.
3. The usual reason of patients choice when doing unnecessary transfer of patient, is highly
discourage, and not allowable.
4. If ever the victim, relative, the offender or any responsible subject insists in doing
unnecessary transfer of their patient, they will be referred to a Private Hospital, and not to a
higher level Government Hospital.
TRIAGE CLASSIFICATION
Type I (Red Tag) - Victims whose injuries are life endangering and demand immediate
attention. These are the priority patients.
Type II (Yellow Tag) - Victims whose injuries demand definitive treatment in the hospital.
However, the said treatment may be safely delayed without prejudice to ultimate recovery.
Type III (Green Tag) - Victims whose injuries demand definitive treatment in the hospital.
However, the said treatment may be safely delayed without prejudice to ultimate recovery.
Type IV (Black Tag) - Victims who are dead.

IX. MONITORING AND EVALUATION


The implementation of the Central Visayas Health Referral System will be monitored and evaluated
regularly at random by the DOH RO7 Health Facility Development Section team together with
representatives from VSMMC and concerned LGU health facilities. The persons to generate referral
reports are the rural health midwife for barangay health stations; the public health nurse or the rural
health midwife for the rural health units; and the emergency room nurse and ward nurse for
hospitals or as designated by the head of facility.
Monitoring and evaluation reports shall be submitted to the area or district health team, or to the
Provincial Health Office where a Monitoring & Evaluation Team has been organized to review and
assess the conduct of implementation.
The following parameters and indicators can be used to gauge the quality of the referral system:
Efficiency (cost of referral the referral should have the least cost for the health
facilities and clients);
Effectiveness (outcome the referral should address the diagnostic and curative
requirements, prevent complications or worsening of the condition);
Accessibility (ease of referrals refers to the existing conditions i.e., transportation,
communication, etc. that would facilitate the transfer and acceptance of the client);
Appropriateness (conformity with protocol the referral facility makes justified referrals
on a timely basis);
Responsiveness (refers to the ability of the receiving facility to accept and manage
referred clients);
Good inter-personal relationship (number of actively participating facilities).
The referral system shall be tracked down through records, such as checklists, logbooks, and reports.
In particular, important information shall include the following:
Number of Patients referred;
Reasons for referral;
Reason(s) for referral;
Number and list of referring
Number and list of receiving
hospitals;

Leading diagnosis;

A. POLICIES AND GUIDELINES ON MEDICO LEGAL CASES AND DEATH


CEBU PROVINCE
(November 2014)

A. As a general rule, all MHOs/CHOs shall act as medico-legal officers in their municipality/city
in the absence of the provincial/city health medico-legal officer;
B.

All requests for medico-legal examinations must be accompanied by an official request from the
police authorities of the concerned city/ municipality;

C.

Medico-legal request not within the capability of the MHO/CHO concerned should be referred
immediately to the NBI/PNP Crime Laboratory/Pink Room/Tertiary Government Hospitals together
with the corresponding reasons for referral;

D.

In cases where the MHO/CHO of the area concerned is not available, the MHO/CHO of the nearest
municipality/city must perform the requested examination;

E.

All medico-legal cases shall be the responsibility of the MHOs/CHOs, unless the patient would require
the services of the hospital for further evaluation and treatment. During weekends and holidays, the
hospital can attend to medico-legal patients and on the next working day, they shall be referred with
complete Physical Examination findings and signature of the medical officer concerned to the
RHU/CHO for the issuance of medico-legal certificate;

F.

Transport vehicle to fetch the MHO/CHO must be provided by the requesting parties concerned. If
post mortem is conducted in a private setting, the MHO/CHO should be escorted by a police officer.
In the absence of the MHO of the municipality, the MHO of the nearest municipality within the ILHZ
shall act as the medico-legal officer. If the MHO/CHO is not around the cadaver will be brought to the
nearest government hospital and let the medical officer do the post mortem examination and request
for autopsy; a waiver must be signed for refusal of autopsy by two (2) nearest of kin;

G. Medico-legal fees shall be paid to the MHO/CHO based on the implementing rules and guidelines
provided by the Magna Carta of the public health workers;
H.

In some instances where there are no MHOs/CHOs available in the area of ILHZ concerned, the
Provincial Health Officer may, upon prior notice, direct any government physician, preferably with
expertise on the case to perform the required examination. This is however, subject to the
presentation of a certification from the Office of the Local Chief Executive concerned that the
MHO/CHO is not available;

All other policies not included herein in relation to the above mentioned subject matter shall be
referred to the Provincial Health Officer for evaluation and approval and subsequent inclusion in this
general policy guideline on referral of medico-legal cases.

BOHOL PROVINCE
(January 2015)
A.

As a general rule, all Municipal Health Officers/City Health Officer shall act as medico-legal officers in their
municipality/City.
B. All requests for medico-legal examinations must be accompanied by an official request from the police
authorities/mayor of the municipalities or barangay
C. Medico-legal requests not within the capability of MHO concerned shall be immediately referred to the NBI
with corresponding reasons for referral.
D. In cases where the MHO of the area concerned is out-of-town and after all efforts to locate him / her had
been exhausted, the MHO available of the nearest municipality within the Inter Local Health Zone must
perform the requested examination
E. All medico-legal cases shall be the responsibility of the MHOs, unless the patient would require the services
of the hospital for further evaluation and treatment.
F. During weekends and holidays, the hospital can attend to medico legal patients provided that the
responsibility of the hospital is only to attend to the patient and refer them to the MHO for the issuance of
medico legal certificate. The MHO can ask for the hospital records which may be necessary for him to issue
the said certificate.
G. All medico-legal certificates must be signed by attending MHOs, CHOs or hospital resident on duty/
designated medico legal resident. Only cases necessitating hospital management within the capability of the
hospital will be attended in the hospitals. In these cases it shall be the responsibility of the hospital to issue
the necessary medico Legal certificates.
H. All medico-legal records must contain complete data such as Date of Incidence, Time of Incidence, Place of
Incidence, Nature of Incidence and finding including anatomical chart.
I. In the absence of the attending physician, medico-legal certificate may be signed by senior residents and
immediate supervisors, provided that medico-legal data are appropriately recorded in the Medico Legal
Logbook.
J. All certificates of deaths from non-natural causes shall be issued by the MHO of the municipality where the
deaths occurred
K. Medical and medico legal certificates of prisoners shall be issued by the MHO of the municipality where the
prison facility is located.
L. For death occurring in transit, the death certificate must be signed by MHO/CHO of the place where the
event occurred.
M. Blood transfusion may not be given where it becomes a religious issue. A waiver should be signed by the
patient or significant others.
N. All rape cases should be automatically referred to Governor Celestino Gallares Memorial Hospital (WCPU).
O. All medico-legal cases after the incident should be responsibility of the MHOs unless the patient would need
the services of the hospital for further evaluation and treatment.
P. Vehicles to transport the MHO must be provided by the requesting parties concerned.
Q. Medico-legal fee shall be paid to the MHO/ attending physician based on the rates provided by the Magna
Carta of Public Health Workers.
R. In some instances, where there are no MHOs available in the area or ILHZ concerned, the Provincial Health
Officer may upon prior notice direct any government physician preferably with experience of the case
presented to perform the required examination. This is however, subject to the presentation of a
certification from the Office of Local Chief Executive concerned that there are no available MHOs/CHO
within the ILHZ.
S. The place of the post mortem examination shall be the option of the MHO or the examining physician in
consideration of safety of the physician.
T. All other policies not included herein in relation to the above mentioned subject matter shall be referred to
the Provincial Health Officer for the evaluation and approval, and subsequent inclusion in this general policy
guidelines on referral of medico-legal cases.

PROVINCE OF NEGROS ORIENTAL
(December 2014)
A. All MHOs are considered medico-legal officers of their own areas of responsibility.
B.

All requests for medico-legal examinations must be accompanied by an official request from the
Fiscal, Judge or Police authorities of the Municipalities/Cities/Barangays concerned.

C.

Medico-legal request not within the capability of the MHO concerned should be immediately referred
to the NBI together with corresponding reasons for referral.

D. All medico-legal records must contain complete data such as date, time, place, and nature of
incidence, findings including anatomical chart.
E.
F.

All medico-legal records must be signed by either the attending MHOs, CHOs, hospital medical staff
and approved by the Chief of Hospital.
The attending physician must sign the medico-legal certificate. However, for referrals for further
management, the referred physician must sign another medico-legal certificate.

G. For death occurring in transit, the death certificate should be signed by the MHO/CHO or attending
hospital physician of the place where the patient came from.
H. In the hospital, the death certificate must be issued by the attending Physician.
I.

For medico-legal cases requiring surgery (in the absence of a companion), consent will be signed by
the attending physician/department head (Surgery and OB-Gyne) or COH.

J.

Blood transfusion will not be given where it becomes a religious issue. (Patient should sign waiver).

K.

Rape cases should be handled by MHO/CHO in their areas of responsibility, unless out of town or sick,
in which case, these should be handled by the neighboring MHO/CHO, unless the patient is
admitted, the hospital attending physician should issue the medical certificate.

L.

In cases where the MHO of the area concerned is out of town and after all efforts to locate him/her
have been exhausted, the MHO of the nearest municipality within the DHS/ILHZ must perform the
examination requested. The appropriate medico-legal fees shall be paid to the examining
MHO/CHO/Hospital Physician as provided for by the Magna Carta for Public Health Workers
charged to the requesting LGU or ILHZ common health fund.

M. Transport vehicle to fetch the MHO must be provided by the requesting parties concerned.
N. Medico-legal fees shall be paid to the MHO based on the rates provided by the Magna Carta for Public
Workers.
O. In some instances where there are no MHOs available in the area or DHS/ILHZ concerned, the
Provincial Health Officer may upon prior notice, direct any government physician preferably with
expertise on the case presented to perform the required examination. This is, however, subject to
the presentation of a certification from the Office of the Local Chief Executive concerned that the
subject MHOs are out of town on official business, or otherwise incapacitated.
P.

All other policies not included herein in relation to the above-mentioned subject matter shall be
referred to the Provincial Health Office/HSRA advocates for evaluation and approval and
subsequent inclusion in this general policy guideline on referral of medico-legal cases.

PROVINCE OF SIQUIJOR
(May 2015)
A. As a general rule, all Municipal Health Officer (MHO) shall act as medico-legal officer in their
respective municipality in the absence of a provincial medico-legal officer.
B.

In cases where the MHO of the area concerned is out of town and after all efforts to locate him/her
have been exhausted, the MHO of the nearest municipality within the Interlocal Health Zone (ILHZ)
must perform the examination requested.

C.

In some instances where there are no MHOs available in the area or ILHZ concerned, the Provincial
Health Officer may upon prior notice, direct any government physician preferably with expertise on
the case presented to perform the required examination. This is, however subject to the presentation
of a certification from the Office of the Local Chief Executive concerned that the subject MHOs are
out of town on official business, or otherwise incapacitated.

D. All requests for issuance of medico-legal certificate must be accompanied by an official request from
the Fiscal, Judge or police authorities of the municipalities concerned.
E.

All medico-legal records must have complete data. (TOI/POI/DOI/NOI, Physical findings and to include
the anatomical chart).

F.

All medico-legal records must be signed by the attending MHOs, CHOs, hospital medical staff and
COH. However, for referrals for furthermanagement, the referred physician must sign another
medico-legal certificate.

G. All medico-legal cases during office hours (8am-5pm) should be the responsibility of the MHOs, unless
the patient would need the services of the hospital for further evaluation and treatment.
H. During weekends and holidays, only those medico-legal cases that sustain severe injuries that
needimmediate diagnostic examination, medical intervention and treatmentupon the assessment of
the resident on duty (ROD) will be entertained. All other minor injuries (upon the assessment of the
ROD) will be referred to the MHO on the next working day but initial medical intervention (first aid)
will be rendered in the emergency room.
I. Medico-legal fees shall be paid to the MHO based on the rates provided by the Magna Carta for Public
Workers, and subject to the usual accounting and auditing rules and regulations.
I-A. In a place where there is no Medico-Legal Officer, the Provincial Health Officer, City Health,
Municipal Health Officer, Rural Health Physician, or Medical Officer of an LGU health office or
government hospital who is a duly designated Medico-Legal Officer, and who has undergone
appropriate training for medico-legal services, may be granted Honorarium per Medico-Legal Service
at the following rates, in view of Section 10 of R.A. No. 7305. Said Honorarium serves as token
payment since such medico-legal service is over and above the regular workload.

I.B. If a designated Medico-Legal Officer needs to travel outside of the official work station in the
performance of medico-legal service, he/she may be allowed to incur expenses relative to local travel
pursuant to the pertinent provisions of Executive Order (E.O.) No. 248 issued on May 29, 1995, as
amended by E.O. No. 248-A issued on August 14, 1995, and E.O. No. 298 issued on March 23, 2004,
and as amended.

RAPE CASES
A. All rape cases must be examined and treated by an Obstetrician and gynecologist (OB/GYNE) in
the hospital. In the absence of an OB/GYNE, the chief of hospital(COH) will examine the patient.
In the absence of an OB/GYNE and COH, the MHO of each respective municipalityshall perform
the examination in the hospital(pending the availability of a laboratory and medical technologist)
and perform the cervico-vaginal smear.

Cervico
Vaginal smear for presence of spermatozoa is a routine procedure in all rape cases.

CONDUCT of Post Mortem Examination


A. For non-hospital cases, post mortem examination should be handled by:
MHO
MHO of nearest or adjacent municipality within the ILHZ
Medical Officer designated by the PHO/COH
In some instances where there are no MHOs available in the area or ILHZ concerned, the Provincial
Health Officer may upon prior notice, direct any government physician preferably with expertise on
the case presented to perform the required examination. This is, however subject to the presentation
of a certification from the Office of the Local Chief Executive concerned that the subject MHOs are
out of town on official business, or otherwise incapacitated.

B.

For Hospital cases:

For DOA (Dead on arrival: BP-0/0, RR-0, pupils dilated), the Resident on duty (ROD) shall
perform post mortem examination and record the findings in the chart and issue the medicolegal certificate but the MHO must issue the death certificate.
For ER Death (with vital signs, pupils not dilated and resuscitation was given but not
successful), attending physician should conduct the post mortem examination and issue the
medico-legal certificate.
For OR Death, attending physician should conduct the post mortem examination and issue
the medico-legal certificate

Guiding Principle: To Refer the Right Patient to the Right Facility at the Right Time All the Time

WHY MAKE THE CALL?
The patient will be thoroughly assessed for the purpose of referral
Referrals sent to other health facilities will be appropriate and properly screened because of prior
consultation and discussion
Referring facilities can be advised on the management and disposition of cases that can be treated and
managed at their level without sending patients for referral
Necessary preparations in the treatment and management of referred patients by the referring and referral
facilities can be made
Anticipated events and transport needs addressed
Savings on economic cost of unnecessary or inappropriate referrals and those managed thru phone referrals
Networking strengthened
Patient and community confidence established on the networked and coordinated healthcare delivery
system, specifically the management of patients thru the HRS
IMPORTANT REMINDERS:
**All Health workers and Stakeholders are expected to maintain proper decorum and extend utmost courtesy and respect at all times
to co-health workers, patients and significant others

**The patients interests, benefits and safety are at the CENTER of all these activities
