Though the Affordable Care Acts intent was to provide health

insurance to many in our nation, it is not and was never intended
to be universal health care coverage. Approximately, 6% or
23,600 community residents of Waukesha remain uninsured in
2015. Many more are underserved and struggling with their
healthcare costs even with having insurance. Deductibles and copays particularly for the chronically ill along with insurance
premiums can be a juggling act for community residents with
increasing costs for food, childcare, medications, housing, and
transportation.
St. Josephs Medical Clinic, Inc. began its mission in providing
healthcare services for the underserved and uninsured 38 years
ago. A local group of proactive and forward-thinking community
leaders undertook a grassroots effort to open the clinic. They
were able to anticipate broad-based community health needs so
that the clinic can continue to serve its mission as a critical safety
net, healthcare provider today. Though state and federal laws
addressing the nations healthcare system shift, so do the
emerging medical care gaps. Nation-wide, free and charitable
clinics have the organizational readiness, expertise and prudence
to adapt and address the evolving community health needs. St.
Josephs is one of the longest-standing independent free and
charitable clinics in the state of Wisconsin, and leverages its
community partnerships and resource navigation in providing
individuals and families with compassionate and quality
healthcare.
This would not be possible without the partners who have
continued so generously, and with foresight, to donate their time,
talents, and financial contributions towards the success of our
mission. In this report, we wish to share with you the highlights
throughout the past year, and to gift you with the knowledge that
your contribution makes a powerful and lasting impact to our
friends and neighbors in need.

2014 Highlights

A total of 834 on-site services were provided including medications, clinic

appointments, and coordination of healthcare access
Individuals served reside in a wide geographic area of Waukesha County
547 clinic appointments serving 393 unduplicated patients
79 medical referrals to network providers
469 lab tests were completed on clinic site
436 lab and diagnostic tests provided off site by network providers
927 prescriptions were provided to 394 patients
Medical Case Management for homeless families and individuals served
19 county residents
Through the Diabetes Waukesha Wellness Working Together Grant SJMC
served 10 Spanish speaking patients
102 patients were supported by the Waukesha County Community Block
Development Grant to obtain their medications
361 patients received medication assistance through support of clinic
42 classes were provided to the public and the community on topics such
as foot care, diabetic self-care, breast health and dental care
898 phone or walk in referrals were received to help residents access
and navigate needed community resources
169 individuals were provided assistance in enrolling in health insurance
through BadgerCare or private insurance on Healthcare.gov.
68 enrolled in Badger Care and received further assistance accessing care
