Reason for Referral:

The test was given to give this student practice administering the Kaufman Test of
Academic Achievement.

Background Information:
This examiner gave this assessment to E. E is a twenty-one year old female. She is in
her senior year at Samford University and is a psychology major. In high school, she was
in the top of her class and received As and Bs throughout her educational career. E
was diagnosed with Epileptogenesis in her freshman year of college. This diagnosis had a
slight impact on her academic performance while in college. She was prescribed
antiseizural medication, she has since been taken off of the medicine and her grades have
improved.

Behavioral Observations:
E and this examiner had established rapport prior to this testing. E was hesitant to
take this assessment. She had never taken an achievement assessment before and was not
sure what to expect. She expressed to me that she did not want to feel dumb if she
happened to miss any easy questions. During the test, E was a bit distracted while
taking the assessment. She was constantly looking around and fidgeting in her seat. After
each question, she asked me if she got the question right because she was afraid of
getting it wrong. If she got a question wrong or she did not know the answer to the
question she would begin laughing because she was embarrassed. E did not have a
positive reaction to this test.

The assessment given was the Kaufman Test of Academic Achievement. The
KTEA is in its second edition. This assessment is meant to assess achievement in the
areas of reading, math, oral language, and written language. The authors of this
assessment are Alan Kaufman and Nadeen Kaufman. The assessment includes a
comprehensive form or a brief form for the examiner to use.
