Goals

Jan. 25th I began establishing a


therapeutic relationship with my
resident in Millpond when I sat with her
and worked on her crossword puzzle. I
also spoke with her about her books
that she had with her, in her walker. I
continued to grow this relationship the
following week Feb. 2nd. The resident
and I spoke of her life history, school,
parents and siblings, husband and
children. She discussed her brothers
deafness and her ability to sign and
went through the alphabet with me.
We continued to work on her
crossword puzzle as well. I furthered
this relationship again Feb. 9th when I

again about her week, doing her nails
and massaging her sore back when
she complained of pain. I feel that my
resident has appreciated and enjoyed
my company. I also feel I have
established therapeutic relationships
with other residents through helping
with personal care, checking in on

Furthermore I feel I have established
the basis of a therapeutic relationship
with Ashtens resident while helping
her with his personal care and
speaking to him at breakfast each
week.

Sabrina consistently shows evidence of


developing therapeutic relationships by
spending time with the long term care
residents, jumping in when someone needs
assistance and being friendly and kind to all of
the residents. Sabrina takes initiative in doing
anything around the home area and is very
eager to learn new things.

I helped to make the beds of residents


throughout the house each week (Jan.
25th-Feb.23rd), helped residents to
participate in Fun & Fitness each week
as well as helped residents to the
television room.
I assisted with helping residents out of
bed each week outside of my own
residents. I have assisted in transfers
from bed to wheelchair, chair to
wheelchair with a pole(each week with
Asthens resident until rotation), hoyer
lift to transfer a resident from a
commode to an electrical chair
(Feb.2), assisted in transferring
patients in and out of the tub during
bathing (Jan. 25th, Feb. 2nd, Feb.
9th.) as well as helped with a sit to
stand transfer from a wheelchair to a
toilet (Feb. 2). I have also helped

residents throughout all weeks to and


from the dining room and their rooms
Sabrina has performed skills such as assisting
residents out of bed in the morning, helping to
toilet and bathe them and use mobility aids in
transporting residents to and from the dining
hall and around the home area (such as
walkers and wheelchairs). Sabrina has been
able to participate in using mechanical lifts
and devices to move residents in and out of
bed and in the shower and tub room. Sabrina
has been consistently making beds and
assisting the staff in the home area.

techniques relevant to

population

I believe I have achieved this by


asking residents constantly if there is
anything that I can do for them, from
helping them to toilet or for a glass of
water. I have learned basic
assessment techniques in lab such as
manual blood pressure, regular
breathing and heart rate, O2 stats etc
and mini psych assessment. I have
implemented them when discerning
the coherency and comprehension of
residents and their mental status
especially if they have dementia or
alzheimer's.I have done this by
clarifying their level of coherency via
questions such as the date. Feb. 2 my
resident was groggy and confused in
the morning and I made sure to factor
this in during transfers and walking.
During this week Ashten and I also
practiced vitals on each of our
residents and went around the house
to collect the vitals of other residents
for the monthly charts.
Sabrina has had the opportunity to do vitals
with her instructor and did very well. Sabrina
is hesitant at first but her values are very close
to the values her instructor gets. Sabrina is
able to assess the residents she has worked
with by asking them how they are doing and
spending time communicating with them.
Sabrina painted her residents nails, did a
crossword with her and was taught the

alphabet in sign language by her. Over the last


half of the semester I expect Sabrina will begin
to use her assessment skills being taught in lab
and apply them in the long term care setting.

Jan 25th I helped with the peri care,


dressing and transferring of Asthens
resident, then helped to serve
residents their breakfast. I helped to
bathe a resident, dress her and blow
dry and brush her hair and toileted a
resident. Feb 2nd I again helped serve
breakfast and I also helped a resident
with feeding who can feed himself with
some encouragement and assistance.
I again bathed a resident and helped
with transferring. Feb. 9th I helped my
resident get dressed and with her
personal hygiene and nail care later in
the day. As well as helped Ashten to
redress and clean up her residence
after he was incontinent in his brief
and it made a mess. Feb. 23rd I
attempted to do a full bed bath on a
resident who was not very content,
cleaned dentures, helped to supervise
another resident in performing his
personal hygiene as dress himself. I
also helped a resident after she was
finished toileting to pull her brief and
pants up and get back into bed. I also
helped a different resident with her
dentures, hearing aid, glasses and to
get dressed.
Sabrina has consistently demonstrated skills in
supporting ADLs. She has been able to get the
residents up in the morning and assist with
and/or perform morning care and toileting.
She has demonstrated the skill of feeding
residents, bathing them in the tub and in the
shower and beginning to do vital signs.

about the experience of
residents living in a long-

I have discovered a deep respect for


the word that goes into caring for the
elderly. I appreciate all that the PSWs

term care setting

and nurses do much more now that I


have seen them in action and helped
them. I have also struggled with my
personal loss of family members,
specifically my grandfather. I have
attempted to discover a little bit about
each resident I speak with, to better
understand them and the reasons they
are currently in long term care. I have
done this each week with each
interaction with different residents. I
have also spoken with specific
residents in depth about their past and
current feelings of being in long term
care (Feb. 2nd, 9th & 23rd).
Sabrina has developed knowledge about the
experience of the residents living in a long
term care home by spending time with them
and developing good therapeutic
communication skills. Sabrina is eager to jump
in and help and is very friendly and outgoing
with the residents.

Demonstrated safe and


ethical clinical practice at
the level appropriate for
a year one nursing
student

During our orientation on Jan. 19th we


we toured the facility and oriented
ourselves a bit and went over policies
and procedures that are followed. We
also did lift and transfer training . Jan.
25th, Feb 2nd, Feb 9th and Feb 23rd
we did report in the morning. I
constantly ensured I was performing
hand hygiene especially before and
after doing personal care and feeding.
I ensured proper body mechanics
when transferring as well as ensured
the residents were properly equipped
to transfer (i.e shoes, having their
wheelchair ready etc). Feb. 23rd when
changing a bed a resident had soiled I
ensured I sanitized the mattress. Each
week I ensured that linens, residents
clothing and garbage was separated
into the correct bins. When a resident
was on isolation (Feb. 2nd) I ensured I

donned and doffed all of the correct


PPEs and disposed of them. I also
reported the pain or confusion and on
one occasion a fall that almost
occurred when a resident was trying to
transfer and swayed back suddenly, to
a resident to a PSW or nurse (Feb. 9th
& 23rd)

Sabrina is always very safe and cautious. If


there is any doubt or if she is unsure she seeks
assistance from staff or her instructor. Sabrina
applied and removed proper PPE on a resident
that was in isolation even when the staff was
telling her she could just walk in the room.
Sabrina is always respectful and professional
towards the residents and the staff at the longterm care home.

Each week in my reflection I go over


all that occurred during clinical. This
allows me to determine where I may
have made mistakes, what I can do to
avoid them in the future or improve on
etc. I also ask both the PSWs and the
nurse questions when I find I do not
know the answer for example where
things are located, how to go about
doing a specific residents personal
care, how to do the snack cart etc.
Sabrina participates in post conference
discussions and asks questions when unsure.
She hands in her weekly reflective note,
however 2/5 of them have been handed in late
and this is unacceptable. Sabrina did not hand
in her midterm until Thur Mar 3, 2016. 6 days
late. I spoke with Sabrina about late
assignments on Tuesday March 1, 2016 and
she understands that if anything else is handed
in late she will not be successful in year 1
clinical.

Examined personal
attitudes regarding the
elderly and other
residents of long-term
care homes

I have discovered that the


stereotyping of older individuals that
society perpetuates arent always
correct and though some residents
may be irritable, I know that seeing
their current way of life and knowing
some of their history, that I would

probably be a tad grumpy if I were in


their position. Often the individuals
who were the most independant before
coming into long term care are
frustrated by their inability to continue
their independence in the same
manner. I also realized that the elderly
are not the only individuals in long
term care and that it is important to
apply the necessary nursing practices
to these other individual and that the
aging process of someone who is in
their 20s is different than that of
someone in their 80s.

knowledge of the clinical
manifestations and
relevant nursing
interventions of chronic
diseases

Jan 25th I looked into the diagnosis of


my resident and worked on and
submitted a care card which included
relevant nursing interventions. I
learned how to fill my residents oxygen
tank and how to change her from her
nighttime oxygen machine to her
portable one. I ensure that when I
communicated with her throughout the
three weeks that if she seemed
confused (dementia related) or said
something that she clearly believed to
be correct but was not, that I did not
correct her or upset her. Feb 23rd I
gathered the information on the
comorbidities of my new resident. I
have experienced some negativity and
resistance from her which has pushed
me to try different methods of
encouragement to help her
complete/do her personal care.
Sabrina has completed and handed in on time
her first care card. This was done very well
and another one will be due in 2 weeks on the
current resident. This information will allow
Sabrina to start to understand disease
processes and develop critical thinking skills
when assessing her residents.

Sabrina is progressing at the expected year 1 level, unfortunately she is not successful at midterm due to her late
assignments. She is very friendly and helpful on the home areas. Sabrina is eager to try new things. In the next 6
weeks I would like to see you applying the assessment skills that you are learning in lab to your residents and
reporting the information to me and trying to tie things together along with diagnoses. Please ensure that ALL
assignments are handed in on time. As per our conversation on Mar 1, 2016, if another assignment is late you will
not be successful at final and this would be very unfortunate because you are doing well in all other areas.
