before entering the patients room, after changing the patient and after leaving the patient's room. Some policies that I still continue to
follow is, wearing gloves, introducing myself to the client and ensure that my patient is the right patient when giving out medication.

S.K.B. : I have demonstrated professional behaviour by assisting colleagues that needed the assistance. One of my colleague needed
assistance with moving up her patient. This task is considered unsafe, since the patient was overweight, and will place strain onto the
student nurse if she did a one person lift. I assisted the student nurse by doing a two-person lift in order for less strain to be placed onto
one person.

S.K.B. : I understand the role of the nurse within the health care. I understand that we are responsible for every action that is done on the
patient. Every medication giving to the patient, must be documented. Every adverse effect that the patient has, it must be documented and
reported to the charge nurse. Every abnormal vital sign must be taken into consideration and reevaluated, and lastly, the client should be
my first priority, I am responsible of the well-being of the patient and I should not disrespect or build a dishonest bond with my client.

after the procedure was done, and also documenting and getting approval by my shadow nurse.

the first introduction and before giving medications. I get an idea of who the client is, what their diagnosis is, how they feel and more. This
information is used when giving report to the other nurses or doctors that want to know about the clients allergies, and how they feel.

S.K.B. : Communication techniques are used with my patients. I listen to my patients concerns, problems and try to resolve the situation.
For example, a patient was complaining about the room temperature. She stated that the room was cold, and she would like to be room. As
a caring nurse, I listened to her concern, and i compromised with the client by asking what she wanted and what she would prefer to have. I
brought a blanket for the client, in order for her to stay warm.

If a client decides to be a smoker, and smoking is bad for them. I have to accept the fact that if the clients decides to do so, and thats is
their choice. I will have to work around it.

did not have a lot of experience with patients that had beliefs and values that wanted to be shown in their care.

E.W. : This means that self awareness of your own personal beliefs and experiences did not interfere with those of your patient. Exampleif you once had a negative experience as a patient in the hospital, then you would not discuss this and try to influence your patient. Or,
your religious beliefs should be kept to yourself and not be brought into the nurse-patient relationship. I think you met this indicator.

she was thirsty, As a student nurse I stated to the client all the types of liquid we had, and made her choose what drink to have, instead of
me picking one out for her. This allows the client to build a bond with me and to know that their is a person to help her out.

S.K.B. : Respond appropriately to unsafe, unacceptable and unprofessional behaviours. An example of unprofessional behaviour is
demonstrated at my clinical placement when a healthcare provider was provided care for the client wrong. She did not placed a soaker pad
underneath the patient while changing the patient's underwear, and was unsure how to take out sutures out. As a nursing student, i went
over to the healthcare provider and example to her how I learned to properly change a patient and demonstrated the right way in order to
avoid infection spread.

Shante developed some leadership skills throughout this clinical placement. She reported her OR experience to the group. She received
feedback well from her clinical teacher and completed a self and peer review. Shante collaborated well with the staff and her clinical
teacher regarding the care for her clients.

S.K.B. : I prioritize my care and interventions in the morning by collecting all of my patients and ensuring what I care I would want to do
first. From knowledge based, I know that insulin is a very important factor that works when its giving at the right time. As a nursing
student, I know that insulin should be the first care I should focus on, In order for it to work on the pancreas at the time of breakfast.

S.K.B. : I have collaborated with the members of the inter-professional health care team when determining the health care that the client
needs. In week 11, I had a patient with one of my colleagues, that had a oxygen saturation (O2 STAT) of 92 %. Me and my colleagues
decided to lift the head of the bed into a high fowler position, and told the patient to do deep breathing, and coughing. After 30 mins, we rechecked his O2 Stat and his respiration and his O2 stat was 95%. We repeated to the patient to do more deep breathing and coughing and
we checked his oxygen saturation again.

Shante has met this outcome and has provided some examples from her practice. She submitted a learning plan with goals to improve
wound dressings, IV meds and sq injections. This can be practiced in the open skills lab. Shante did receive feedback well and would try to
incorporate it the next week in clinical.

S.K.B. : Critical thinkings and problem-solving skills to inform decision-making in all aspects of nursing care is done in my clinical
placement by advocating for my client and making sure they are my main priority. This is done by organizing the plan of care of the clients.
Vital signs first, to get a base line and base on the vital signs plan the care for the client. For example if patient is on sliding scale and the
patients blood sugar is above the norm, medication would have to be prepare.

determine nursing diagnoses, expected outcomes, nursing interventions, and evaluative

S.K.B. : Critical thinking, problem solving and decision making skills are used to assess my client in several ways. Vitals signs help with
this factor as it tells the homeostasis of the client. If a client has abnormal blood sugar, that alerts me to call the MD if it is way out of range,
and provide medication stat to the patient and monitor the patients vitals to avoid any other complications that can occur. Another factor
that vital signs can help with is abnormal respirations. By having abnormal respirations ( like respiration under 12) or an oxygen saturation
under 93% alerts me that the patient is not breathing. I must provide oxygen ( only 2L, unless have a doctors order), lift the head of the bed
to high fowlers or sitting so the patient has a better airway passage to breathe.

S.K.B. : I evaluate my skills each time that they are done in order to improve better on them. Some decision making that i designed to
improve better with my care is the use of sterile field. I try to improve not to contaminate the sterile field as the inside of the bod needs
sterile objects to operate on to avoid infection. What I can do is prepare my sterile field and all my clean equipment before hand to avoid
having to turn my back on the sterile field or walking away. Which will lead to having to have a new sterile tray available.

S.K.B. : Use knowledge from nursing textbooks and nursing videos in order to provide safe competent care. From my nursing textbooks I
apply procedures that are demonstrated in the textbook. Removal of staple and sutures must be done with gloves on, and removed in a
way that the material that has been exposure on the outside of the skin does not get pulled back into the skin to cause infection.

Shante successfully completed this surgical placement. Over the 14 weeks caring for two patients, she developed her critical thinking skills,
prioritization skills, communications skills and advanced in the other outcomes. I urge Shante to learn to write better English grammar by visiting the
Writing Centre. Also, keep practicing to pronounce the medications by breaking the work into smaller syllables. Over this semester I have observed
