NUR4948L Preceptorship Evaluation
As the semester comes to an end we value your input as the clinical preceptor and will use
your comments in the students final semester evaluation. If you can take a minute to write
a brief narrative addressing these headings we will incorporate into the final evaluation.

Motivation to Learn:
Ashley was always quick to get in and learn whatever she could. Any new experience she would
jump right in and try to experience it first hand.

Patient Assessment

She was thorough in assessing her patients and documented appropriately to convey the patients
condition.

Communication

She would appropriately communicate any concerns she had about the patients to me. She would
also explain to the patients exactly any activity she would be performing on them before she did
it (assessments, wound care, ect.).

Skills
She performed nursing skills well and would ask appropriate questions if she wasnt sure how to
perform a skill she wasnt very familiar with.
Safety
She was aware of possible medication errors and tried to apply every safety measure she could
while passing meds and taking care of the patients.
Documentation
Her documentation appropriately described the patients condition. She just needed a little more
coaching on being a little more descriptive with the patient and actions that were being taken to
address the issues.

Professionalism
She was always very professional when interacting with the patients and the other nurses.
