M.T.H. : I demonstrated a good client relationship on a therapeutic level in clinical settings with all patients. This was done by introducing
myself, referring to patient by using their preferred name. I also respect each patient culture and their beliefs.

M.T.H. : During clinical one of the conflict that arises was a patient who refused his medication including a schedule insulin. After
providing health teaching the nurse was contacted.

M.T.H. : Care plans that were created were used to help provide care to clients. This care plan involved achievable goals. Care plans were
then utilized to make sure that outcomes were met. They were then evaluated to see the effectiveness of care plans.

M.T.H. : Plan of care are created based on theory from textbooks and researched done on clients' diagnosis. This information is then

M.T.H. : Knowledge from textbooks, patient charts, progress notes and lab results were use to prioritize clients' needs and outcomes. For
example: Each time when giving warfarin i have to make sure i check the patient's lab results for INR. The INR was then assess to see if it
was safe to administer the medication.

M.T.H. : When carrying out head to toe assessments on clients a safe practice that was incorporated was privacy and correct positioning
of the bed. Nursing interventions such as suctioning a patient is done in a safely manner such as ensuring the i maintained sterility of the
first six inches of the suctioning catheter to avoid contamination. Patient is given oxygen in between suctioning to keep their oxygen from
becoming low.

M.T.H. : I carry out nursing care that reflects the practice decision-making framework. Our Nursing Theory 1 class taught us how to carry

M.T.H. : As a student nurse on the unit all my responsibilities such as head to toe assessments, gathering vital signs, giving medications,
providing hygienic care, feeding, were all done in a timely and organized manner. For example: vitals were the first thing to be done when i
start my shift.

M.T.H. : Consultation were done on a daily basis in clinical to ensure that care is centered around the client. For example: A patient was
not supposed to regularly suctioned but the nurse who was in charged had not completed documentation as to how long a go was the
patient suctioned, so I consulted to make sure it was okay for me to suction the patient. Another example was when i was changing a
different kind of inner cannula, I seek assistance from my teacher to guide me and make sure i was doing it correctly.

closed to maintain a room temperature. When it got warmer I asked patients if they wanted the windows opened.

M.T.H. : Satisfactory in this area because I made sure i leave beds in the lowest position and raised said side rails. Call bell is always
within reach and any unsafe equipments that could pose a threat to patient are out of reach, and all necessary items that patient need are
also within reach. If I am unsure about doing something for a patient such as giving patient tylenol for pain i consult with the nurse.

opportunity. Feedback was then asked from my clinical instructor and nurse that was present to help with my development in this area.

M.T.H. : The way some of the nurses and my clinical teacher carry out their nursing practice were use as guide to help me develop my
nursing skill such as time management. Patient health were asses each week to improve or maintain optimal health. Information from our

medications that needed to be taken at the same time. I assessed both patients medications and then decide to administer anti-diabetic
medications first.

For example: when giving medications I gave the patient on diabetic medication before my other patient.
