H.N. : During my placement at Credit Valley Hospital, I have collaborated with my nursing instructor, fellow students and nursing staff to
ensure that I comply with the regulations created by the College of Nurses (CNO), Humber College and Trillium Health Partners. I
understand that I am obligated to be compliant, knowledgable and informed about these various standards in order to practice as a nurse. I
have learned that these standards are in place to protect the public, nurse, nursing profession and institution. I am aware that if I do not
adhere to these regulations, I jeopardize the health and safety of the patient. This semester, I learned many valuable lessons pertaining to
the importance of following all regulations created by the college and institution. Since, I have continued to work diligently to ensure I meet
these standards. When necessary, I review the Humber PN Student Handbook and various CNO practice and professional standards to
further my knowledge and understanding. In future semesters, I will continue to demonstrate competence regarding the various
regulations created by the CNO and Humber College. I will continue to review and utilize the resources provided to ensure compliance. I
will develop further knowledge pertaining to my obligations and expectations as a nurse. I will collaborate with my future instructor to
ensure I am meeting this criteria.

interventions, I have demonstrated that I am responsible and accountable for those actions. This includes any possible consequences
associated with these decisions and actions. Particularly, taking accountability for errors and ensuring corrective measures are taken. I
continuously assess my limitations and capabilities for providing nursing care. When a limitation is identified, I collaborate with my

care team including but not limited to physiotherapists, social worker, respiratory therapists, pharmacists and clinical nurse leaders.

obligation to protect the patient's confidentiality and privacy. I do not communicate information about the client's health with personnel
outside of the clinical setting. This includes the patient's mental health, physical health, family history, and care provided. I ensure that I
protect the personal health information in compliance with PHIPA by removing any identifiers such as name from documents I take outside
of the clinical setting.
