The provisions of the prospectus must be read carefully before applying for admission.

Application Form, complete in all respects ,must be accompanied by the following:

a)

Four latest passport size photographs of the duly candidate attested by Header master/Principal.

b) Form B issued by NADRA must be attached.

c)

Undertaking on the accompanying (Serial 5) signed by the candidate and countersigned by the
parent / guardian.

d) Address slip (Given at the end of the Prospectus as Appendix D duly completed).

Incomplete Application Form, the Application Form of those who are overage/underage and Application
Forms unaccompanied by any of the documents listed above not be entertained.
