COMES NOW, Petitioner, through the undersigned counsel,

unto this Honorable Court, most respectfully states that:

1. Under Section 5, paragraph (4) of A.M. No. 02-11-10-SC, Petitioner is

required to serve a copy of the Petition on the Office of the Solicitor
General and the Office of the City or Provincial Prosecutor, within five
days from the date of its filing and submit to the court, proof of such
service within the same period.

2. That it was on December 14, 2015 that Petitioner learned that his case
was raffled to RTC Branch __.

3. By sheer inadvertence, however, a copy of the Petition was not

immediately furnished to the Office of the Solicitor General (OSG)
and the Office of the City Prosecutor (OCP) within the period
required. It is important to note, however, that all succeeding
pleadings were furnished to the OSG and the OCP.

4. In compliance with the said rule, a copy of the Petition is hereby

furnished to the OSG, which copy is herewith attached for the OSG.

5. Also, it is manifested that the OCP is hereby furnished a copy of the

Petition, which copy is herewith attached for the OCP.

WHEREFORE, undersigned counsel hereby manifests that she

has complied with the A.M. No. 02-11-10-SC to furnish a copy of the
Petition to the Office of the Solicitor General and the Office of the
Prosecutor.
