Subject:- Appointment for the post of Staff Nurse

With reference to your application looking for opportunity of training as a apprentice in
our organization and your subsequent interview, we are pleased to take you as an
apprentice in our organization on the following:

Terms and Conditions:

1. Your apprentice period shall be for a period of 6 months in the first instance which
can be extended further or terminated earlier without giving any notice or
assigning any reason thereof.
2. You will be paid stipend of Rs.__________/month
No other allowances as admissible to regular employees will be paid to you.
3. You shall be abide by the instruction of your superior in matters pertaining to the
training as well as the rules of discipline either existing or extended from time to
time.
4. While there is no guarantee of regular employment to be offered to you completion
of the apprenticeship period, the management might consider your case for regular
absorption after successful completion of training on any vacancy on such terms
and conditions as may be agreed upon.
5. Any breach or violation of any instructions/rules will render you liable for
termination of the apprenticeship without assigning any reason or notice.
6. You shall observe punctuality and learn the trade in the working place diligently.

7. Your services are liable to be transferred to any place, department, and

any branch existing or coming into existence in near future.
8. You will be required to maintain a daily performance diary, regarding
your engagements and achievement of day to day training and produce the
same to your superior as and when so required.
9. You shall not be entitled to claim any other privileges, benefits which are
available to the other employees except the stipend payable during the
apprenticeship period.
10. You are directed not to go for a long leave i.e. more than a week during 1st Year
of your service period.
11. While deciding to leave the organization or at the time when you resign from the
services. One month prior notice shall be mandatory or the salary for one
month will be surrendered.
12. In case the above terms and conditions are acceptable to you, please sign the
duplicate copy in token of your acceptance.
13. After completion of 6 months of apprenticeship you would move a fresh
application through your in-charge for extension, which may or may not be
accepted.
