GENERAL INSTRUCTIONS:1

The appraisal forms needs to be filled in the excel sheet and forwarded to your respective
Reporting Manager's. No print outs of the same would be allowed.

The review forms contain confidential information and should be treated with due respect for the
individual's privacy. Any violation to the same would be viewed strictly.

After the annual review meeting, your Reporting Manager would share his review report with you,
so that you understand the improvement areas needed.
Review Period for associates who has been in the company for more than a year would be 01 April
YYYY to 31st March YYYY. And for the rest it would be from Date of Joining to 31st March
YYYY.Employees who have joined the company after 31st December YYYY would not be eligible for
the appraisal.
The appraisal form should contain sufficient information and analysis to support the Self and
Manager Ratings. Incomplete forms or forms with inadequate data will be sent back for a rereview.
The space marked in pink(Tan shade) is to be filled by the Reporting Manager's only
The space marked in yellow is to be filled by the Employee.
The last Sheet marked in Pink is for HR use only and not to be filled either by the individual or the
manager.
For filling in section III of the form please refer the attached 'Attributes Directory'.
TIME LINE
DD/Month to DD / Month --- Associates to complete their respective Performance Review forms.
DD / Month / YYYY--- The duly Filled PRF to reach the respective Reporting Managers on or
before DD / Month / YYYY.
DD / Month / YYYY --- Reporting Managers to have performance review meetings with team
members individually and complete the ratings.
DD / Month / YYYY --The duly filled forms by the Reporting Manager should reach HR on or before DD / Month /
YYYY.
Forms reaching beyond the target date (i.e.
) would delay the appraisal cycle of the entire
branch/unit by a month.
