Diagnosis:

BIPOLAR DISORDER most recent episode of manic phase with psychotic features

Pertinent Data:

A case of patient x, 45 years old, from Guada lupe Cebu city, female, single, unemployed,
Filipino, Roman Catholic, admitted for the second time at Vicente Sotto Memorial Medical
Center- Center for behavioral Sciences (VSMMC-CBS) on August 27,2008 due to “ stone
throwing” incident along their residential premise aimed at their neighbors as reported by the
father.

Patient’s grandfather was diagnosed of a psychiatric illness9 diagnosis not indicated on


chart). Her maternal side has positive hypertension and diabetes mellitus. Patient has
previously hospitalized at VSMMC-CBS last 2003( diagnosis not indicated on chart).
Patient is an occasional alcohol beverage drinker, a non-cigarette smoker, and has no
allergies on drugs and sea foods.

Following Terms:

1. BIPOLAR I – the patient experiences swing between manic episodes and major
depression.

2. Bipolar II – Similar to Bipolar I disorder with the major exception being that the
person has never experienced a manic episodes but only hypo manic episodes.

3. MANIA – A mood characterized by an unstable expansive emotional state, extreme


Definition of excitement, excessive elation, hyperactivity, agitation, over talkativeness, flight of
Terms: ideas, increase psychomotor activities, fleeting attention and sometimes violent,
destructive and self-destructive behavior.

4. MAJOR DEPRESSION - A major disorder of mood characterized by a persistent


dysphoric mood, anxiety, irritability, fear, brooding, appetite and sleep disturbances,
weight loss, psychomotor agitation or retardation, decrease energy, feelings of
worthless or guilt, difficult in concentrating or thinking, possible delusion and
hallucination and thoughts of death or suicide.

5. EUHORIA – a feeling / state of wellbeing or elation. An exaggerated or abnormal


sense of physical and emotional well being not based on reality or truth,
disproportionate to its case, and in appropriate to the station.

6. MOOD SWING – an oscillation between period of well-being and depression.
