1.CLIENTS PERSONAL BACKGROUND

Hj. Abas Bin Buntar is a 77 year old Malay patient. He is a pensioner and lives with his only son at Lambak Kanan, BSB. He has seven children, one son and six daughters. In his family, he is the only son out of seven sisters. He is seems to be a quiet patient, however, when approached or spoken to, he always reply in a friendly manner.

Though at the age of 77 years old, Hj. Abas is still able to walk and do activities such as gardening. Before Hj. Abas was admitted he stayed at his sons house where he is an active gardener. He also plants fruits and vegetables. He would frequently ask about the condition of his plantation at home as he worries his family doesnt take good care of his plants.

CLIENTS PAST MEDICAL AND SURGICAL HISTORY Hj. Abas has a medical history of Chronic Obstructive Pulmonary Disease (COPD), high blood pressure, asthma and also known case of high lipids. He has several surgical history since 2009. On 11th October 2009, he went to Accident and Emergency Department complaining of pain at the super pubic area. The next day,12th October 2009 had cystoscopy, urethral dilation, bladder washout, Trans-urethral insicion of the prostate under s.a.

On the 22nd October 2009, he had another surgery,Laparotomy, sigmoid colectomy, Hartmans procedure under general anesthesia. Then on the 24th December 2009, he had mutual dilation and cystoscopy under s.a.
