NOTICE OF FAILURE TO FILE A


FINANCIAL DISCLOSURE STATEMENT

As a "State officer or employee" holding a "policymaking" position as designated by your


appointing authority, you are required to file an Annual Statement of Financial Disclosure. The
records of the State Ethics Commission indicate that you have failed to file the financial disclosure
statement for calendar year 1995 which was due either May 15) 1996 or within 30 days of
appointment to your pollcymaking position. Failure to file a financial disclosure statement is a
violation of law.

You have fifteen days from the date ofthis Ngtice within which to file ygur financial disclosure
statement with the Commission. If you fail to file within 15 days) the Commission win issue a
