APPLICATION DOCUMENTARY REQUIREMENTS 1. Duly accomplished SOSIA Application Form with 2x2 ID picture (PNPSF01-2005) 2. SSS Number - Applicants with no SSS Number shall fill-up SSS Personal Record Form (E-1) and file the same at any SSS Branch for issuance of SSS Number 3. Duly accomplished SSS Social Security ID Application Form (E-6) 4. Birth Certificate 5. High School Diploma 6. Training - Any of the following:

Pre-licensing Training Certificate (Authenticated by C, Records Branch) Basic ROTC/CMT Certificate Retirement Order - for retired PNP/AFP Personnel Discharged Order - for former PNP/AFP personnel honorably discharged from the Service

Filipino citizen Holder of a Baccalaureate Degree Physically and mentally fit Has graduated from a Security Officer Training Course Retired personnel of the PNP or AFPI Must possess one of the following qualifications: Holder of Masters degree in either Criminology, Public Administration, MNSA, Industrial Security Administration or Law o Must have at least ten (10) years experience in the operation and management of security business.

Filipino citizen High school graduate Physically and mentally fit At least eighteen (18) years of age but not more than fifty (50) years old Has undergone pre-licensing training course Must possess one of the following qualifications: o Holder of a baccalaureate degree or Bachelor of Laws o Holder of a degree of Bachelor of Science in Criminology o Graduate of a Criminal Investigation Course offered by the then Philippine Constabulary/ Integrated National Police or the Philippine National Police or the National Bureau of Investigation or any police training school or detective training with authorized/ recognized training center for security guard. o Advance ROTC/CMT graduate.
