Physical Assessment Body Part Skin Inspection Normal Findings - Skin is Uniform whitish pink or brown color - No Bleeding

- No Area of increased Vascularity and Echymosis - There is a lesions present except for birthmarks or moles which may be flat or elevated - Skin is dry with scales at upper and lower extremeties. Actual Findings Interpretation and Analysis

Skin is brown. No bleeding


No Area of Echymosis Lesions are present in upper extremeties Skin is dry with scales often located at upper and lower extremeties. Warm and equal bilaterally

Palpation

- Warm and Equal bilaterally - Hands and skin slightly cooler than the rest of the body - Skin surfaces non-tender - Texture: Smooth, even and firm - Skin Turgor: should return to original contour rapidly - Edema not present - Dark black to pale, may turn gray or white, may be chemically changed - Terminal hair fiund in eyebrows, eyelashes, axilla and pubic areas. - No signs of infestation or lesions. - Seborrhea or dandruff may be present. - Hair may feel thin, straight course, thick or curly, shiny and resilient - Brown cast in dark skinned - Capilliary Refill present (2-3 seconds)

Skin sprung back rapidly when pinched No edema present Hair appears black

Even distribution of hair No Lesion present

Palpation

Hair is thick and shiny

Nails

Inspection

Cardio Edema Orthopnea + Grade 2 Bipedal Edema Non smoker, Heavy alcoholic drinker + crackles on the left mid-based lung fields
