You been reported absent for (9) days since March 12, 2012, without submitting your medical certificate, although you advised your superior on the first day of your absent. The next day we called you to update on your condition, you said you are on process of your check up, we also informed you to submit your medicate certificate to justify your current health condition but you failed to send it. Its your responsibility to inform and give notice on your superior about your situation so they can make a necessary action on the work. This misconduct is a violation of Section H Paragraph 5 ABSENTEEISM (that unexcused absence (5) consecutive days or more without justifiable reason) is immediately subject for dismissal. Wherefore, it is hereby decided that your employment with Eagle Star is terminated effective March 21, 2012.
